超声与磁共振成像在乳房无症状硅胶假体内破裂中的应用研究

摘　　要：目的探讨超声(US)和磁共振成像(MRI)诊断乳房无症状硅胶假体内破裂的价值。方法采取前瞻性研究,选择30例因美容原因而置入乳房硅胶假体的无症状患者作为研究对象。以手术结果为金标准,计算US和MRI术前诊断假体内破裂的灵敏度、特异度、阳性似然比、阴性似然比、相对危险度及准确度。结果US和MR术前诊断乳房无症状硅胶假体内破裂的灵敏度、特异度、阳性似然比、阴性似然比、相对危险度和准确度分别为80.00%和80.00%,70.00%和92.50%,2.67和10.67,0.29和0.22,4.57和8.59,73.33%和88.33%。虽然MRI的准确度高于超声(83.33%vs.73.33%),但两者的诊断效能差异无明显统计学意义(P=0.09)。结论US与MRI对乳房无症状硅胶假体内破裂具有较高的诊断价值,两者的诊断效能差异无统计学意义。Objective To investigate the diagnostic value of ultrasonography(US)and magnetic resonance imaging(MRI)in asymptomatic intracapsular rupture of a silicone implant.Methods A prospective study was conducted including 30 asymptomatic patients requiring silicone implant implantation for aesthetic purposes.US and MRI evaluation results were compared with intraoperative findings.Sensitivity(SE),specificity(SP),positive likelihood ratio(LR+),negative likelihood ratio(LR-),relative risk(RR)and accuracy(AC)of US and MRI imaging in detecting intracapsular rupture implants were calculated.Results The SE,SP,LR+,LR-,RR and AC of ultrasound and MRI were 80.00%and 80.00%,70.00%and 92.50%,2.67 and 10.67,0.29 and 0.22,4.57 and 8.59,73.33%and 88.33%respectively.MRI imaging performed better than US for diagnosis of breast implant intracapsular rupture,with overall accuracy of 83.33%vs.73.33%,respectively,but the difference between US and MRI were not significant(P=0.09).Conclusions US and MRI have high diagnostic value for asymptomatic intracapsular rupture of a silicone implant.There is no significant difference in the diagnostic value between US and MRI.
