摘  要:目的分析低速血流成像(SlowFlowHD)与频谱多普勒超声对诊断胎儿生长受限的效能。方法前瞻性收集就诊的晚孕期单胎妊娠孕妇92例,根据指南发布的相关诊断标准,分为胎儿生长受限组(FGR,26例)和正常胎儿组(66例)。对两组均使用SlowFlowHD检测胎盘内血管结构、测定胎盘微血管血流参数,频谱多普勒超声测定脐动脉、大脑中动脉的收缩期最大血流速度和舒张末期血流速度比值(S/D),统计各组胎儿的检出情况,并绘制受试者工作特征(ROC)曲线分析各参数对FGR胎儿的诊断效能。结果应用SlowFlowHD显示正常组胎盘微血管清晰而血流丰富、呈树枝状分布,而FGR组血流稀少、呈点线状分布;SlowFlowHD的检出率为90.64%,优于频谱多普勒超声;通过ROC曲线分析,血管指数和血流指数预测FGR胎儿的曲线下面积为0.858、0.833,大于脐动脉和大脑中动脉的S/D值曲线下面积(0.746、0.719)。结论SlowFlowHD技术对胎盘微血管结构呈树枝状的层级显示,预测FGR胎儿的能力更高,可以作为一种有潜力的工具为临床诊断妊娠相关疾病提供新思路和新方法。Objective To analyze the efficacy of SlowflowHD and spectral Doppler ultrasound in the diagnosis of fetal growth restriction.Methods A prospective collection of 92 pregnant women with singleton pregnancies in late pregnancy were prospectively collected,and they were divided into the fetal growth restriction(FGR)group(26 cases)and the normal fetal group(66 cases)according to the relevant diagnostic criteria published by the guidelines.SlowFlowHD was used to detect the vascular structure in the placenta,the placental microvascular blood flow parameters were measured,the peak systolic velocity/end diastolic velocity(S/D)values of the umbilical artery and the middle cerebral artery were measured by spectral Doppler ultrasound,the detection of fetuses in each group was counted,and the receiver operating characteristic(ROC)curve was plotted to analyze the diagnostic performance of each parameter on the fetuses with FGR.Results The application of SlowFlowHD showed that the placental microvessels in the normal group were clear and abundant,and the blood flow was abundant and branch-like,while the blood flow in the FGR group was sparse and point-like.The detection rate of SlowFlowHD was 90.64%,which was better than that of spectral Doppler ultrasound.Through ROC curve analysis,the area under the curve of vascularization index(VI)and flowi ndex(FI)predicted FGR fetus was 0.858 and 0.833,which was larger than that of the umbilical artery and middle cerebral artery(0.746,0.719).Conclusions With the use of SlowFlowHD,the placental microvascular structure is dendritic and this sign has a higher ability to predict FGR fetuses,which can be used as a potential tool to provide new ideas and methods for the clinical diagnosis of pregnancy-related diseases.
