摘　　要：目的探讨睾丸间质细胞瘤的多模态超声声像图特点及临床表现。方法回顾性分析经手术病理证实的21例睾丸间质细胞瘤患者的声像图资料,包括灰阶超声、彩色多普勒超声、超声造影和压力应变式弹性成像。结果灰阶超声:21例患者肿瘤最大径(13.929±12.179)mm;21例(100%)病灶表现为实质低回声;20例(95.24%)病灶内部回声不均,1例(4.76%)病灶内部回声均匀;5例(23.81%)边界不清晰,16例(76.19%)边界清晰。彩色多普勒超声:21例(100%)病灶内可探及2级以上丰富的血流信号。11例患者超声造影检查发现,9例(81.82%)总体表现为“快进慢退”,1例(9.09%)总体表现为“快进快退”,1例(9.09%)总体表现为“不典型快进慢退”。5例患者病灶的压力应变式弹性成像评分平均为(3.200±0.837)分。结论睾丸间质细胞瘤的多模态超声声像图表现具有一定的特征,应用多模态超声成像有利于提高睾丸间质细胞瘤的诊断准确度。Objective To investigate the multimodal ultrasound sonographic features and clinical manifestations of testicular Leydig cell tumor.Methods Retrospective analysis of the sonographic data,including gray-scale ultrasound,color Doppler ultrasound,contrast enhance ultrasound(CEUS)and pressure-strain elastography,in 21 patients with testicular Leydig cell tumor confirmed by surgical pathology was performed.Results The characteristics of gray-scale ultrasound showed that,the maximum diameter of the tumor was(13.929±12.179)mm;all the cases showed parenchymal hypoechoic lesions;20(95.24%)had uneven internal echogenicity,1(4.76%)had homogeneous internal echogenicity;5(23.81%)had unclear borders,and 16(76.19%)had clear borders.In color Doppler ultrasound,rich blood flow signals of grade 2 or higher could be detected within the lesion in 21 cases(100%).The findings in 11 patients who received CEUS showed that,9 cases(81.82%)had“fast in and slow out”pattern,1(9.09%)showed“fast in and slow out”pattern,and another 1(9.09%)showed“fast wash-in and fast wash-out”pattern."The mean stress-strain elastography score of the lesions in 5 patients was(3.200±0.837).Conclusions The multimodal ultrasound sonograms of testicular Leydig cell tumor have certain characteristics,and the application of multimodal ultrasound imaging is beneficial to improve the diagnostic accuracy of testicular mesenchymal stromal cell tumors.
