桃红四物汤合健脾丸与循坐骨神经穴位针刺在老年全髋关节置换术的应用  

Application of Taohong Siwu Decoction Combined with Jianpi Pill and Sciatic Nerve Acupoint Acupuncture in Elderly Total Hip Arthroplasty

摘　　要：目的探讨桃红四物汤合健脾丸与循坐骨神经穴位针刺在老年全髋关节置换术(THA)患者中的应用效果。方法选取2022年10月—2023年10月湖口县中医院收治的80例老年THA患者,采用随机数字表法分为观察组和对照组,各40例。2组均行常规康复治疗方法,对照组采用桃红四物汤治疗,观察组采用桃红四物汤合健脾丸与循坐骨神经穴位针刺,比较2组中医证候积分、术后恢复情况、髋关节Harris评分量表得分、傅格-梅尔氏运动量表(FMMS)得分以及康复治疗的总有效率。结果治疗后,观察组主症、次症的中医证候积分低于对照组(P<0.05),术后下床活动时间、骨折愈合时间以及住院总时长均小于对照组(P<0.05),Harris评分与FMMS评分均高于对照组(P<0.05),康复治疗的总有效率高于对照组(P<0.05)。结论在老年THA患者中,桃红四物汤合健脾丸与循坐骨神经穴位针刺疗法具有较好的干预效果,能够改善患者症状,加快术后康复速度,提升康复疗效,且在改善髋关节功能及下肢运动能力方面有一定的优势。Objective To explore the application value of Taohong Siwu decoction combined with Jianpi pill and sciatic nerve acupoint acupuncture in elderly total hip arthroplasty(THA).Methods Eighty cases of elderly THA patients admitted to Hukou Hospital of Traditional Chinese Medicine from October 2022 to October 2023 were selected and grouped into two groups using the random number table method,with 40 cases in each group.Both groups were treated with conventional rehabilitation intervention programmes,the control group was treated with Taohong Siwu decoction,and the observation group was treated with Taohong Siwu decoction combined with Jianpi pill and sciatic nerve acupoint acupuncture.TCM syndrome score,postoperative recovery,Harris hip scale score,Fugger-Mayer Movement Scale(FMMS)score and total effective rate of rehabilitation treatment were compared between the two groups.Results The TCM syndrome scores of the main and secondary diseases in the observation group were lower than those in the control group(P<0.05),the time of getting out of bed after surgery,the time of fracture healing and the total length of hospital stay were lower than those in the control group(P<0.05),the Harris score and FMMS score were higher than those in the control group(P<0.05),and the total effective rate of rehabilitation treatment was higher than that in the control group(P<0.05).Conclusion In elderly patients with THA,Taohong Siwu decoction combined with Jianpi pill and sciatic nerve acupoint acupuncture has a better intervention effect,can reduce the TCM evidence score,accelerate the speed of postoperative recovery,enhance the total effective rate of rehabilitation therapy,and has certain advantages in improving hip function and lower limb mobility.
