摘  要:目的分析2013—2020年广州市疾病预防控制中心收集多粘菌素耐药基因(mediated colistin resistance,mcr-1)阳性鼠伤寒沙门菌的基本流行病学情况、耐药特点、分子特征与亲缘关系。方法使用VITEK 2 Compact全自动微生物生化鉴定仪进行菌株鉴定。使用PCR和琼脂糖凝胶电泳对mcr-1阳性菌株进行筛选。应用微量肉汤稀释法和脉冲场凝胶电泳法对mcr-1阳性菌株进行药敏试验和分子分型实验。对mcr-1阳性菌株进行全基因组测序和系统发育分析。结果2013—2020年,在分离的1783株鼠伤寒沙门菌中共筛出mcr-1阳性菌64株。各年鼠伤寒沙门菌的mcr-1阳性率为0.37%~6.84%。药敏结果表明64株mcr-1阳性菌株均对多粘菌素耐药,对氨苄西林、链霉素和四环素的耐药率分别为98.44%(63/64)、96.88%(62/64)和96.88%(62/64),而对阿米卡星、美罗培南和阿奇霉素耐药性分别为3.12%(2/64)、3.12%(2/64)和10.94%(7/64)。PFGE结果表明64株阳性菌株按约70%的同源性进行划分为5个同源性较高的聚类。系统发育树进一步提示不同来源mcr-1鼠伤寒沙门菌其传播路径的多样性和复杂性。结论广州市近年来mcr-1阳性鼠伤寒沙门菌高检出率高,且耐药谱多样化,不同菌株间遗传多样性较高,应继续加强耐药监测与溯源。Objective To analyze the basic epidemiological characteristics,drug resistance profiles,molecular characterization features,and phylogenetic relationships of mcr-1(polymyxin resistance gene,mediated colistin resistance)-positive Salmonella typhimurium collected by Guangzhou Center for Disease Control and Prevention(GZCDC)from 2013 to 2020.Methods Strain identification was performed using a VITEK 2 Compact fully automated microbial biochemical identifier.The mcr-1-positive strains were screened using PCR and agarose gel electrophoresis.The mcr-1-positive strains were subjected to drug sensitivity tests and molecular typing experiments by micro broth dilution and pulse-field gel electrophoresis(PFGE).Whole genome sequencing and phylogenetic analysis were performed on the mcr-1-positive strains.Results From 2013 to 2020,64 mcr-1-positive strains were screened out of 1783 Salmonella typhimurium strains,and the mcr-1 positivity rate ranged from 0.37%to 6.84%.The drug sensitivity results showed that all 64 mcr-1 positive strains were resistant to polymyxin and exhibited resistance to≥3 classes of antibiotics,with 90.63%(58/64)of strains resistant to≥7 classes.The PFGE results indicated that the 64 positive strains could be grouped into 5 clusters with approximately 70%homology.The phylogenetic tree further suggested the diversity and complexity of transmission pathways of mcr-1-positive Salmonella typhimurium from different sources.Conclusions There has been a high detection rate of mcr-1-positive Salmonella typhimurium in Guangzhou City in recent years,along with a diverse drug resistance spectrum and high genetic diversity among different strains.Continued monitoring and tracing of drug resistance should be strengthened.
