负压创口治疗与传统敷料治疗下肢创伤开放性骨折的比较

摘　　要：目的比较负压创口治疗(NPWT)与传统敷料治疗下肢创伤开放性骨折的疗效。方法回顾性分析2021年1月-2023年9月南阳文和骨科医院收治的下肢创伤开放性骨折患者120例,采用随机数表法随机分为对照组(传统敷料治疗,60例)和NPWT组(NPWT,60例),比较两组治疗效果、创口严重程度、疼痛、炎性反应。结果NPWT组创口体积缩小率、创口愈合率高于对照组(P<0.05)、创面感染率、创口愈合时间低于对照组(P<0.05);两组治疗1 d、7 d后压疮愈合(PUSH)评分、视角模拟评分(VAS)、高敏C反应蛋白(hs-CRP)、WBC水平低于治疗前(P<0.05),研究组PUSH评分、VAS评分、hs-CRP、WBC低于对照组(P<0.05);结论与传统敷料治疗下肢创伤开放性骨折比较,NPWT可降低创面的感染率,促进创口缩小和愈合,减轻创口严重程度和疼痛,降低炎性反应。Objective To compare the effects of negative pressure wound therapy(NPWT)and traditional dressings on the treatment of open fractures of lower limb trauma.Methods From January 2021 to September 2023,120 patients with open fractures of lower limb trauma were selected and divided into a control group(60 cases treated with traditional dressings)and a study group(60 cases treated with NPWT)using a random number table method.The treatment effects,wound severity,pain,and inflammatory reactions of the two groups were compared.Result The rate of wound volume reduction and wound healing in the NPWT group was higher than that in the control group(P<0.05),and the rate of wound infection and wound healing time were lower than that in the control group(P<0.05).After 1 d and 7 d of treatment,the levels of pressure ulcer healing(PUSH)scores,VAS scores,hs-CRP,and WBC were lower than those before treatment in both groups(P<0.05),and PUSH scores,VAS scores,hs-CRP,and WBC were lower than those in the control group in the study group(P<0.05).Conclusion Compared with traditional dressings for open fractures in lower extremity trauma,NPWT reduces the infection rate of the wound,promotes wound reduction and healing,reduces wound severity and pain,and inflammatory response.
