基于“肺合皮毛”及“肺与大肠相表里”理论探讨从肠论治艾滋病皮肤瘙痒

摘　　要：艾滋病皮肤瘙痒是人类免疫缺陷病毒(HIV)/获得性免疫缺陷综合征(AIDS)患者最常见的并发症之一,常给其带来极大的身心压力。高效联合抗反转录病毒疗法(HAART)虽能大幅降低HIV/AIDS患者的病死率,但对艾滋病皮肤瘙痒疗效不佳,甚至还会引发相关药疹。肠道菌群为近年来研究的热门话题,已有相关研究表明其与HIV/AIDS之间存在密切联系,中医药能够调节肠道菌群从而改善艾滋病相关症状。因此,该文基于“肺合皮毛”及“肺与大肠相表里”理论,探讨从肠论治艾滋病皮肤瘙痒的可行性,拓宽艾滋病皮肤瘙痒的临床诊疗思路。AIDS-related pruritus is one of the most common complications in patients with human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS),often causing significant physical and psychological stress.While highly active antiretroviral therapy (HAART) can significantly reduce the mortality rate in HIV/AIDS patients,its efficacy in alleviating AIDS-related pruritus is limited,and it may even induce drug-related rashes.The gut microbiota has become a hot topic of research in recent years,with studies showing a close relationship between gut flora and HIV/AIDS.Traditional Chinese Medicine (TCM) has been found to regulate the gut microbiota,potentially improving symptoms related to AIDS.Therefore,based on the theories of‘lung associating with skin and hair’and‘lung and large intestine being interior-exterior related’,this paper aims to explore the feasibility of intestinal treatment for AIDS-related skin pruritus,thereby broadening the scope of clinical diagnosis and management strategies for AIDS-related skin pruritus.
