摘  要:目的探讨利用3D可视化技术及3D打印骨折模型微创治疗髌骨骨折的临床价值。方法选取2021年12月至2022年12月九江市中医医院收治的40例髌骨骨折患者作为研究对象,按照入院床位奇偶数分为3D打印组与对照组,每组20例。3D打印组利用3D可视化技术及3D打印骨折模型进行髌骨骨折微创治疗,对照组采用传统切开复位克氏针张力带固定术。比较两组手术情况(手术切口长度、手术时间、术中出血量)及术后不同时间患肢肿胀程度、疼痛程度[视觉模拟评分法(visual analogue scale,VAS)评分]、骨折愈合情况及关节功能(Bost-man评分)和并发症发生情况。结果3D打印组手术切口长度、手术时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。术后3、6个月,两组VAS评分均低于前一时间点,骨折愈合情况、Bostman评分均高于前一时间点,差异有统计学意义(P<0.05);术后6个月,对照组肿胀程度低于术后1、3个月,3D打印组低于术后1个月,差异有统计学意义(P<0.05);术后1、3、6个月,3D打印组肿胀程度、VAS评分均低于对照组,Bostman评分均高于对照组,差异有统计学意义(P<0.05);术后1、3个月,3D打印组骨折愈合情况评分均高于对照组,差异有统计学意义(P<0.05),两组术后6个月骨折愈合情况评分比较差异无统计学意义。两组并发症发生率比较差异无统计学意义。结论利用3D可视化技术及3D打印骨折模型微创治疗髌骨骨折,可有效减小手术创伤,缩短手术时间,减少术中出血量,有利于膝关节的功能恢复,且不增加并发症的发生,具有较高临床应用价值。Objective To explore the clinical application value and treatment advantages of minimally invasive treatment of patellar fracture using 3D visualization technology and 3D printing fracture model.Methods 40 patients with patellar fracture admitted to Jiujiang Traditional Chinese Medicine Hospital from December 2021 to December 2022 were selected as research subjects,and they were divided into the 3D printing group and the control group according to the odd even number of hospital beds,with 20 cases in each group.The 3D printing group was treated with 3D printing technology combined with patellar fracture reduction and hollow nail internal fixation,while the control group was treated with traditional open reduction and Kirschner wire tension band fixation.The operation conditions(incision length,operation time and intraoperative bleeding),degree of limb swelling and pain(visual analogue scale[VAS]score),fracture healing,joint function(Bostman score)at different time points after operation,and complications were compared between the two groups.Results The incision length,and operation time in the 3D printing group were shorter than those in the control group,intraoperative bleeding was less than that in the control group,the differences were statistically significant(P<0.05).At 3 and 6 months after operation,the VAS score of the two groups were lower than those at the previous time point,and the fracture healing and Bostman score were higher than those at the previous time point,and the differences were statistically significant(P<0.05);at 6 months after operation,the swelling degree of the control group was lower than that of 1 and 3 months after operation,and the 3D printing group was lower than that of 1 month after operation,and the difference was statistically significant(P<0.05);at 1,3,and 6 months after operation,the swelling degree and VAS score in the 3D printing group were lower than those in the control group,and the Bostman score were higher than that in the control group,and the differences were statist
