摘 要:目的探讨多普勒超声检查在胎儿生长受限(fetal growth restriction,FGR)与小样儿诊断中的应用价值。方法选取2020年12月至2022年2月于赣州市妇幼保健院产检的80例疑似FGR及小样儿产妇作为观察组,另选取本院同期产检的80名正常妊娠产妇作为对照组,两组均进行多普勒超声检查。以分娩结局为金标准,分析多普勒超声检查与分娩结果,比较两组超声指标和FGR及小样儿与正常儿超声指标升高率。结果80例疑似FGR及小样儿产妇中,多普勒超声检查确诊FGR与小样儿51例,确诊率为63.75%。观察组脐动脉搏动指数(umbilical artery pulsatility index,UA-PI)、子宫动脉搏动指数(uterine artery pulsatility index,PIUTA)均低于对照组,大脑中动脉收缩期峰值流速(middle cerebral artery peak systolic velocity,MCA-PSV)快于对照组,差异有统计学意义(P<0.05);FGR及小样儿UA-PI及MCA-PSV升高率分别为100.00%、56.86%,均高于正常儿24.14%、13.79%,差异有统计学意义(P<0.05);FGR及小样儿与正常儿PIUTA升高率比较差异无统计学意义。结论多普勒超声诊断通过动态观察UA-PI、PIUTA、MCA-PSV变化了解胎儿生长趋势,有助于检出FGR与小样儿,具有一定临床应用价值。Objective To explore the application value of Dopplerultrasonography in diagnosis of fetal growth restriction(FGR)and small sample infants.Methods 80 parturients with suspected FGR and small sample infants who underwent prenatal examination in Ganzhou Maternal and Child Health Hospital from December 2020 to February 2022 were selected as the observation group,and 80 normal pregnant women whounderwent prenatal examination in our hospital during the same period were selected as the control group,both groups underwent Doppler ultrasonography,took delivery outcome as the gold standard,the results of Doppler ultrasonography examination and delivery were analyzed,the ultrasound indexbetween the two groups,and the increase rate of ultrasound index between infants with FGR and small sample and normal infants.Results Among 80 suspected parturients with FGR and small sample infants,51 cases with FGR and small sample infants were confirmed by Doppler ultrasonography,the diagnosis rate was 63.75%.The umbilical artery pulsatility index(UA-PI),uterine artery pulsatility index(PIUTA)in the observation group were lower than those in the control group,and median middle cerebral artery peak systolic flow velocity(MCA-PSV)was faster than that in the control group,the differences were statistically significant(P<0.05).The increasing rates of UA-PI and MCA-PSV in FGR and small sample infants were 100%,56.86%,respectively,which were higher than 24.14%and 13.79%in normal infants,the differences were statistically significant(P<0.05);there were no significant differences in the increase rate of PIUTA between FGR and small sample infants and normal infants.Conclusion Doppler ultrasonography diagnosis can understand the trend of fetal growth through dynamically observing the changes of UA-PI,PIUTA and MCA-PSV,which can help to detect FGR and small sample infants,and has certain clinical application value.
