摘 要:目的探究超声引导的真空辅助微创旋切术治疗乳腺纤维瘤的临床效果以及对患者术后康复的影响。方法选择2021年4月至2024年2月期间平顶山市第一人民医院乳腺外科收治的224例乳腺纤维瘤患者进行研究,根据患者接受的手术方式将患者分为常规组(114例)和微创旋切组(110例),前者开展传统乳晕切口术,后者开展超声引导下的真空辅助微创旋切术。对比两组患者的病灶残留情况、围术期指标、术后3 d及术后6个月的乳晕区神经功能情况、术后并发症发生情况及对手术相关内容的满意度水平。结果两组患者术后病灶残留比较,差异无统计学意义(P>0.05);但微创旋切组患者手术切口长度、瘢痕长度、手术时长及住院时长均明显短于常规组,且术中出血量也明显少于常规组,差异具有统计学意义(P<0.05)。术后3 d两组患者的乳晕区神经功能情况比较,差异无统计学意义(P>0.05),术后6个月微创旋切组患者乳晕区神经功能情况优于常规组,且微创旋切组术后6个月较术后3 d神经功能改善情况变化比较,差异具有统计学意义(P<0.05)。微创旋切组患者术后并发症发生率明显低于常规组,且该组患者对手术各方面的满意度明显优于常规组,差异均有统计学意义(P<0.05)。结论超声引导下真空辅助微创旋切术治疗乳腺纤维瘤的临床疗效较理想,并且患者术后康复较快,术后并发症发生率较低,患者满意度较高。Objective To investigate the clinical effects of ultrasound-guided vacuum-assisted minimally invasive excision for the treatment of breast fibroadenoma and its impact on postoperative recovery.Methods A total of 224 patients with breast fibroadenoma treated at the Department of Breast Surgery of Pingdingshan First People's Hospital from April 2021 to February 2024 were selected for the study.Patients were divided into two groups based on the surgical method:the conventional group(114 cases)underwent traditional periareolar incision surgery,while the minimally invasive excision group(110 cases)received ultrasound-guided vacuum-assisted minimally invasive excision.The two groups were compared in terms of the residual lesions,perioperative indicators,nerve function in the periareolar area at 3 days and 6 months postoperatively,incidence of postoperative complications,and satisfaction with surgical aspects.Results There was no statistically significant difference in the residual lesions between the two groups postoperatively(P>0.05).However,the minimally invasive excision group had significantly shorter incision length,scar length,operation time,and length of hospital stay compared to the conventional group,with significantly less intraoperative blood loss(P<0.05).At 3 days postoperatively,there were no significant differences in nerve function between the two groups(P>0.05);however,at 6 months postoperatively,nerve function in the periareolar area was better in the minimally invasive excision group compared to the conventional group,and the improvement in nerve function from 3 days to 6 months postoperatively was statistically significant in the minimally invasive excision group(P<0.05).The incidence of postoperative complications in the minimally invasive excision group was significantly lower than that in the conventional group,and satisfaction regarding various aspects of the surgery was significantly higher in the minimally invasive excision group compared to the conventional group(P<0.05).Conclusion Ultrasound-
