手术治疗对绝经后期重度OSAHS患者主观睡眠质量的影响

摘　　要：目的探讨手术治疗对绝经后期重度阻塞性睡眠呼吸暂停患者主观睡眠质量改善的有效性和相关影响因素。方法回顾性分析34例进行手术治疗的绝经后期重度OSAHS患者,术后随访2年。对患者术前临床数据,及手术前后匹斯堡睡眠量表(PSQI)、Epworth嗜睡量表(ESS)评分进行数据收集和统计分析。结果患者PSQI及ESS评分显著低于术前(P<0.01),PSQI评分改善程度与扁桃体大小、t_(90%)负相关(P<0.01);ESS评分变化与Friedmen分级(P<0.01)、t_(90%)负相关(P<0.05),与扁桃体大小(P<0.01)、MRS评分正相关(P<0.05)。结论绝经后期OSA患者睡眠质量差,手术治疗可以改善睡眠质量与白天嗜睡症状,是一个可行的选择。扁桃体大小、t_(90%)、Friedmen评分MRS评分和是影响术后睡眠质量的重要风险因素。Objective To explore the effectiveness and risk factors of surgery in improving sleep quality of postmenopausal patients with severe OSAHS.Methods A retrospective analysis was made on surgical cases of 34 postmenopausal patients with severe OSAHS.The patients were followed up for 2 years.The patients'preoperative clinical data,and pre-and postoperative Pittsburgh Sleep Quality Index(PSQI)and Epworth Sleepiness Scale(ESS)scores were collected and statistically analyzed.Results PSQI and ESS scores were significantly lower than those before operation(P<0.01).The improvement of PSQI score was negatively correlated with tonsil size and t_(90%)(P<0.01).The change of ESS score was negatively correlated with Friedmen grade(P<0.01)and t_(90%)(P<0.05),and positively correlated with tonsil size(P<0.01)and MRS score(P<0.05).Conclusions Postmenopausal patients with OSAHS have poor sleep quality,surgical treatment can be a viable option to improve sleep quality and daytime sleepiness symptoms.Tonsil size,MRS score,Friedmen score and t_(90%)were significant risk factors of postoperative sleep quality improvement.
