超重肥胖男性青少年脂代谢特点及性激素的相关性分析

摘  要:目的 探讨超重肥胖男性青少年脂代谢异常、体脂率的特点,探讨与雌二醇、睾酮的相关性关系.方法 选取就诊于我院内分泌代谢科及体检科13-17岁男性青少年53例,根据体质指数(BMI)进行分组,超重肥胖男性青少年31例为观察组,对照组为同年龄范围的正常体重男性青少年22例,记录受试者体重,计算BMI,记录首次遗精年龄,测定血清总胆固醇(Total Cholesterol,TC)、甘油三酯(Triglyceride,TG)、低密度脂蛋白胆固醇(Low Density Lipoprotein Cholesterol,LDL-C)及雌二醇(Estradiol,E2)、睾酮(Testosterone,T),完善体脂率(Body Fat Percentage,Fat%)检查,并进行相关性分析.结果 ①观察组的Fat%、TG、LDL-C、TC均明显高于对照组.②观察组血脂异常比例高达54.84%,其中以LDL-C升高为主,Fat%均明显升高,均值高达32.1%,明显高于标准值.③观察组与对照组间,TC、LDL-C、TG、Fat%均存在显著的统计学差异(P<0.01).④观察组与对照组的性激素比较,发现两组间E2及T差异无统计学意义(P>0.05),两组首次遗精年龄无统计学差异(P>0.05).⑤男性超重肥胖组Fat%与E2呈正相关关系(r=0.66,P=0.022),与T无明显相关性(P>0.05),血脂各项指标与性激素无明显相关性(P>0.05).结论 超重肥胖男性青少年发生脂代谢异常比例高,以LDL-C升高为主,体脂率亦明显升高,高Fat%可引起超重肥胖男性青少年雌激素异常升高,可能影响青春期的性发育.Objective To investigate the characteristics of abnormal fat metabolism and body fat percentage in overweight and obese male adolescents,and to explore the correlation between estradiol and testosterone.Methods Fifty-three male adolescents aged 13-17 years old who were admitted to the Department of Endocrinology and Metabolism and physical Examination of our hospital were selected and divided into groups according to Body Mass Index(BMI).31 overweight and obese male adolescents were included in the observation group,and 22 normal weight male adolescents in the same age range were included in the control group.Subjects'weight was recorded,BMI was calculated,age of first ejaculation was recorded,serum Total Cholesterol(TC),Triglyceride(TG)and Low Density Lipoprotein(Low Density Lipoprotein)were measured Cholesterol(LDL-C),Estradiol(E2),Testosterone(T),Body Fat Percentage(Fat%)were examined,and correlation analysis was performed.Results①Fat%,TG,LDL-C and TC in the observation group were significantly higher than those in the control group.②The rate of dyslipidemia in the observation group was as high as 54.84%,in which LDL-C was mainly increased,and Fat%was significantly increased,with an average of 32.1%,which was significantly higher than the standard value.③There were significant statistical differences in TC,LDL-C,TG and Fat%between the observation group and the control group(P<0.01).④The comparison of sex hormones between the observation group and the control group showed no significant difference in E2 and T between the two groups(P>0.05),and no significant difference in age of first spermatogenesis between the two groups(P>0.05).⑤In male overweight and obese group,Fat%was positively correlated with E2(r=0.66,P=0.022),but not significantly correlated with T(P>0.05),and there was no significant correlation between lipid indexes and sex hormones(P>0.05).Conclusion Overweight and obese male adolescents have a high rate of abnormal Fat metabolism,mainly with increased LDL-C and significantly increased bo
