摘  要:目的 探讨可注射富血小板纤维蛋白(injectable platelet-rich fibrin, i-PRF)联合脂肪间充质干细胞(adipose-derived mesenchymal stem cells, ADMSCs)修复兔耳软骨缺损的可行性。方法 自2023年6—8月,江苏省苏北人民医院医学美容科从临床腹部抽吸获得的颗粒脂肪组织中提取原代脂肪间充质干细胞体外传代培养。并通过流氏细胞技术及三系分化细胞鉴定。选取8只新西兰大白兔,均于双耳对称处设计直径0.9 cm的圆形全层软骨缺损,每2只兔子的4只耳郭分别作为A组(对照组):0.5 ml生理盐水;B组(i-PRF组):0.5 ml i-PRF;C组(ADMSCs组):5×106个ADMSCs;D组(i-PRF联合ADMSCs组):0.5 ml i-PRF+5×10^(6)个ADMSCs,各组分别于术后当天,术后第2天,术后第4天予以相应处理。28 d后处死实验兔,取出耳软骨标本,进行形态学、组织病理学分析。结果 观察可见:A组软骨缺损处由肉芽样组织填充,B组、C组可见缺损周围光滑的轮廓样新生软骨组织,D组可见光滑的新生软骨组织几乎填充整个缺损区。组织病理学结果显示,A组软骨缺损区域软骨组织边界清晰,几乎无新生软骨组织形成,可见大量纤维组织填充缺损区域。B组、C组可见软骨基质包裹一定数量的软骨细胞,C组新生软骨细胞多于B组。D组可见最大面积的新生软骨组织,与自体软骨残端连续排列,填充软骨缺损区域,新生软骨组织内可见致密的软骨基质包绕的软骨细胞。结论 i-PRF联合ADMSCs可用于修复兔耳软骨缺损。Objective To investigate the feasible method of repairing rabbit ear cartilage defects by injectable platelet-rich fibrin(i-PRF)combined with adipose mesenchymal stem cells(ADMSCs).Methods From June2023 to August 2023,adipose mesenchymal stem cells were extracted from the adipose tissue of patients with the tummy tuck for in vitro passaging culture.The cells were identified by flow cytometry and trilineage differentiation.Eight New Zealand Large White rabbits were selected,all of which were designed with a full-layer cartilage defect with a diameter of 0.9 cm at the symmetrical place of both ears,and the four auricles of every two rabbits were used as Group A(control group):0.5 ml saline;Group B(i-PRF group):0.5 ml i-PRF;Group C(ADMSCs group):5×106ADMSCs;Group D(i-PRF combined with ADMSCs group):0.5 ml i-PRF+5×10^(6)ADMSCs,each group was treated correspondingly.28 days later,the ear cartilage defects were analyzed morphologically and histopathologically.Results In group A,the cartilage defect was filled with granulation-like tissue,in groups B and C,smooth contour-like new cartilage tissue was seen around the defect,and in group D,smooth new cartilage tissue was seen to fill almost the entire defect area.Histopathological analysis showed that in group A,the cartilage defect area had clear cartilage tissue boundaries,no new cartilage tissue was formed,a large amount of fibrous tissue could be seen to fill the defect area,and a certain number of chondrocytes could be seen wrapped in the cartilage matrix in groups B and C,and the number of new chondrocytes in group C was more than that in group B.In group D,the largest area of new chondrocytes could be seen in the area of cartilage defect,and it was arranged continuously with the stumps of the autologous cartilage.Dense chondrocytes surrounded by cartilage matrix were seen within the newborn cartilage tissue.Conclusion i-PRF combined with adipose stem cells can be used to repair cartilage defects in rabbit ears.
