紧密型医联体背景下医防融合慢病管理体系的探讨

摘　　要：医防融合指的是打破公共卫生服务体系和医疗卫生服务体系间的体制壁垒,通过依托信息化、整合公共卫生资源与医疗资源,达到提高卫生资源利用率、改善人群健康状况的目的。然而目前我国医防融合领域有关研究仍较少,各地区在医防融合的开展模式上存在差异,工作开展缺乏相应的标准规范。为完善和优化医防融合慢性病管理体系,南宁市第二人民医院依托广西科技重大项目,以紧密型医联体为平台,通过总体规划、统筹布局、规范流程、信息建设等系列举措,构建了6个常见多发病的医防融合慢性病管理体系。文章就其在构建这一体系中的经验与做法进行介绍,以期为其他医院开展医防融合工作提供参考。Integration of medical care and prevention refers to breaking down institutional barriers between the public health service system and the medical service system to achieve the goals of improving the utilization rate of health resources and the health status of the population by relying on information technology and integrating public health resources with medical resources.However,there are still few studies on integration of medical care and prevention in China and there are differences in the implementation models of the integration among various regions,indicating the lack of corresponding standard specifications.To improve and optimize the integrated medical care and chronic disease management system,the Second Nanning People's Hospital took the opportunity of the Guangxi Science and Technology Major Programs and built an integrated medical care and chronic disease management system for six common and frequently-occurring diseases through a series of measures,including overall planning,coordinated layout,standardized processes,and information construction.This study introduces the experience and practice in building this system to provide references for other hospitals to carry out the same work.
