健康人群胸部后前位摄影质控分析

摘　　要：目的回顾性分析体检中心健康人群胸部后前位摄影重照原因,并对后续工作质控提出整改措施,以提高影像质量和减少误诊率。方法查看河北省人民医院体检中心2023年2月1日至2024年2月1日所有胸部后前位摄影涉及二次曝光的影像,对各种原因引起的重照影像进行分析,找出问题所在后进行总结。结果健康人群1年内胸部后前位摄影体检数据统计共计87334例,确认进行重照的1326例,占总数的1.52%。查阅这些影像排查原因发现:吸气不足共491例、体外异物394例、肺野没包全(中心线左右或上下偏离)、因肥胖将竖向探测器改为横向或需要2次投照的320例、肩胛骨在肺野内的70例、不明原因的15例、摄影对象错误13例、发现可疑病变补拍的13例、因曝光条件不足或中心线上下偏离导致电离室探测位置错误进而使图像过明或过暗的10例,共计1326例。其中吸气不足占第一位,因此投照时的呼吸训练是很有必要的。因影像异物而重照,除了一般的配饰比如项链、吊坠、纽扣、胶印图案、内衣等等,还有容易被检查者忽视的头发,特别是喷了定形、护发产品的头发,更容易显示在图像上,最终影响诊断结果。另外脱掉衣物还可以更好的确定受检者的体型位置和受检者的呼吸幅度来判断定位线。结论在健康人群胸部后前位摄影中,质控是确保影像质量的关键。通过制定严格的质量标准和流程,并对影像操作人员进行培训和监督,可以有效提高胸部后前位摄影的质量水平。同时,还建议结合技术的发展趋势,应更加重视健康人群的辐射检查,减少重照率,不断更新质控标准,及时应用新的影像技术手段和方法,以进一步提高影像质量和临床诊断的准确性。Objective This paper retrospectively analyzes the causes of re-photographing the chest of healthy people in the physical examination center,and puts forward rectification measures for the follow-up quality control to improve the image quality and reduce the misdiagnosis rate.Methods We checked all the images of the back and front chest photography involving the second exposure in the physical examination center of Hebei Provincial People′s Hospital from February 1,2023 to February 1,2024,and analyzed the re-exposure images caused by various reasons to find out the problems and summarize them.Results A total of 87334 cases of physical examination data of chest anteroposterior photography in healthy people within six months were included,and 1326 cases were confirmed to be re-photographed,accounting for 1.52%of the total.By reviewing these images,we found that there were 491 cases of insufficient inspiration,394 cases of foreign body in vitro,320 cases with incomplete lung field(left and right or up and down deviation of the center line),vertical detector changed to horizontal because of obesity,or needed two shots,70 cases of scapula in the lung field,15 cases of unknown reasons,13 cases of wrong exposure,13 cases of suspicious lesions were found and 10 cases of over-bright or over-dark images due to insufficient exposure conditions or up-and-down deviation of the center line.Among them,the lack of inspiration takes the first place,so it is necessary to have breathing training when casting photos.In addition to well-known accessories such as necklaces,pendants,buttons,offset printing patterns,underwear,etc.,there are also hairs that are easily overlooked by inspectors,especially those sprayed with styling and hair care products,which are more likely to be displayed on the image and ultimately affect the diagnosis results.In addition,taking off clothes can also better determine the body position of the subject and the breathing amplitude of the subject to judge the positioning line.Conclusion Quality control is t
