舒腰活血方联合选择性神经根阻滞治疗腰椎间盘突出症临床研究

摘　　要：目的:观察舒腰活血方联合选择性神经根阻滞治疗气滞血瘀型腰椎间盘突出症的临床疗效。方法:选取气滞血瘀型腰椎间盘突出症患者108例,按随机数字表法分为对照组和试验组各54例,2组均剔除2例,最终纳入每组52例。对照组予选择性神经根阻滞治疗,试验组在对照组基础上加用舒腰活血方治疗,2组均治疗2周。比较2组临床疗效、中医证候积分、视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)评分、炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6、IL-1β]水平。结果:治疗后,试验组总有效率94.23%,高于对照组80.77%(P<0.05)。2组中医证候积分、VAS评分、ODI评分均较治疗前降低(P<0.05),试验组中医证候积分、VAS评分、ODI评分均低于对照组(P<0.05)。2组TNF-α、IL-6、IL-1β水平均较治疗前降低(P<0.05),试验组TNF-α、IL-6、IL-1β水平均低于对照组(P<0.05)。结论:舒腰活血方联合选择性神经根阻滞治疗气滞血瘀型腰椎间盘突出症可提高临床疗效,改善中医证候和腰部功能,缓解疼痛,抑制炎性反应。Objective:To observe the clinical effect of Shuyao Huoxue Prescription combined with selective nerve root block on lumbar disc herniation with qi stagnation and blood stasis type.Methods:A total of 108 cases of patients with lumbar disc herniation with qi stagnation and blood stasis type were selected and divided into the control group and the trial group according to the random number table method,with 54 cases in each group.Two cases were excluded from each group,with a final total of 52 cases per group.The control group was treated with selective nerve root block,and the trial group was treated with Shuyao Huoxue Prescription on top of the control group's treatment.Both groups were treated for two weeks.The clinical efficacy,traditional Chinese medicine syndrome scores,visual Analogue Scale(VAS)scores,Qswestry Disability Index(ODI)scores,and levels of inflammatory factors[tumor necrosis factor-α(TNF-α);interleukin(IL)-6,IL-1β]were compared between the two groups.Results:After treatment,the total effective rate in the trial group 94.23%was higher than that 80.77%in the control group(P<0.05).Traditional Chinese medicine syndrome scores,VAS scores,and ODI scores in the two groups were decreased when compared with those before treatment(P<0.05),and the scores in the trial group were lower than those in the control group(P<0.05).The levels of TNF-α,IL-6,and IL-1βin the two groups were decreased when compared with those before treatment(P<0.05),and the levels in the trial group were lower than those in the control group(P<0.05).Conclusion:Shuyao Huoxue Prescription combined with selective nerve root block in the treatment of lumbar disc herniation can enhance clinical efficacy,improve traditional Chinese medicine syndromes and lumbar function,relieve pain,and inhibit inflammatory response.
