督脉灸联合附子理中丸治疗脾肾阳虚型腹泻型肠易激综合征临床研究

摘　　要：目的:观察督脉灸联合附子理中丸治疗脾肾阳虚型腹泻型肠易激综合征(IBS-D)的疗效。方法:采用随机数字表法将60例脾肾阳虚型IBS-D患者分为试验组和对照组各30例。2组均予以蒙脱石散治疗,对照组加用附子理中丸口服,试验组在对照组基础上加用督脉灸,2组均治疗4周。比较2组临床疗效、中医证候积分、大便性状评分、血清炎症因子水平。结果:治疗后,2组总有效率比较,差异无统计学意义(P>0.05)。治疗后,2组各项中医证候评分、Bristol评分均较治疗前降低(P<0.05);且试验组各项中医证候评分、Bristol评分均低于对照组(P<0.05)。治疗后,2组血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平较治疗前降低(P<0.05),且试验组血清TNF-α、IL-6水平低于对照组(P<0.05)。结论:督脉灸联合附子理中丸治疗脾肾阳虚型IBS-D疗效显著,可以改善中医证候和临床症状,抑制炎性反应。Objective:To observe the curative effect of the combination use of moxibustion on governor vessel and Fuzi Lizhong Pills on diarrhea-predominant irritable bowel syndrome(IBS-D)of spleen-kidney yang deficiency type.Methods:A total of 60 IBS-D patients of spleen-kidney yang deficiency type were divided into the trial group and the control group by the random number table method,with 30 cases in each group.Both groups were treated with Montmorillonite Powder;the control group was additionally treated with oral administration of Fuzi Lizhong Pills,and the trial group was additionally treated with moxibustion on governor vessel based on the treatment of the control group.Both groups were treated for four weeks.The clinical effects,traditional Chinese medicine syndrome scores,scores of stool trait and levels of serum inflammatory factors were compared between the two groups.Results:After treatment,there was no significant difference being found in the comparison of the total effective rate between the two groups(P>0.05).After treatment,the traditional Chinese medicine syndrome scores and Bristol scores in the two groups were reduced when compared with those before treatment(P<0.05);the above scores in the trial group were lower than those in the control group(P<0.05).After treatment,the levels of serum interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the two groups were down-regulated when compared with those before treatment(P<0.05),and the above two levels in the trial group were lower than those in the control group(P<0.05).Conclusion:The combination use of moxibustion on governor vessel and Fuzi Lizhong Pills has a significant curative effect on IBS-D of spleen-kidney yang deficiency type,which can improve the traditional Chinese medicine syndromes and clinical symptoms and inhibit the inflammatory responses.
