针刺联合中药督脉熏蒸疗法治疗脾肾气虚兼痰湿内盛型围绝经期肥胖临床研究

摘　　要：目的:观察针刺联合中药督脉熏蒸疗法治疗脾肾气虚兼痰湿内盛型围绝经期肥胖(PMO)的临床疗效。方法:选取80例脾肾气虚兼痰湿内盛型PMO患者,按随机数字表法分为对照组与治疗组各40例。对照组给予针刺治疗,治疗组给予针刺联合中药督脉熏蒸疗法治疗,2组均治疗4周。比较2组临床疗效、中医证候积分、改良Kupperman评分、肥胖程度指标[体质量、体质量指数(BMI)、腰围、臀围、腰臀比指数(WHR)]。结果:治疗后,治疗组总有效率95.00%,高于对照组77.50%(P<0.05)。2组形体肥胖、痰涎壅盛、神疲体倦、头晕、耳鸣或耳聋、性欲减退、喜食肥甘厚味、口干不欲饮、便溏积分及中医证候总积分均较治疗前降低(P<0.05),治疗组上述9项证候积分及中医证候总积分均低于对照组(P<0.05)。2组改良Kupperman评分均较治疗前降低(P<0.05),治疗组改良Kupperman评分低于对照组(P<0.05)。2组体质量、BMI、腰围、臀围及WHR均较治疗前降低(P<0.05),治疗组体质量、BMI、腰围、臀围及WHR均低于对照组(P<0.05)。结论:针刺联合中药督脉熏蒸疗法治疗脾肾气虚兼痰湿内盛型PMO有较好的疗效,可改善围绝经期症状,减轻肥胖程度。Objective:To observe the clinical effect of acupuncture combined with Chinese medicine fumigation at du mai on perimenopausal obesity(PMO)of spleen and kidney qi deficiency and internal abundance of phlegm-damp type.Methods:A total of 80 cases of PMO patients of spleen and kidney qi deficiency and internal abundance of phlegm-damp type were selected and divided into the control group and the treatment group according to the random number table method,with 40 cases in each group.The control group received acupuncture treatment,while the treatment group received acupuncture combined with Chinese medicine fumigation at du mai,both groups were treated for four weeks.Compared the clinical efficacy,traditional Chinese medicine(TCM)syndrome scores,modified Kupperman scores,and obese degree indexes[body mass,body mass index(BMI),waist circumference,hip circumference,and waist hip ratio(WHR)]between the two groups.Results:After treatment,the total effective rate was 95.00%in the treatment group,higher than that of 77.50%in the control groupm(P<0.05).The scores of various symptoms such as obesity,excessive phlegm and saliva,spiritual lassitude and fatigue,dizziness,tinnitus or deafness,hypaphrodisia preference for fatty and sweet foods,dry mouth and unwillingness to drink,and loose stools,as well as the total TCM syndrome scores in both groups were lower than those before treatment(P<0.05).The above nine syndrome scores and the total TCM syndrome score in the treatment group were lower than those in the control group(P<0.05).The modified Kupperman scores in both groups were lower than those before treatment(P<0.05),and the modified Kupperman score in the treatment group was lower than that in the control group(P<0.05).The body mass,BMI,waist circumference,hip circumference,and WHR in both groups were lower than those before treatment(P<0.05),and the body mass,BMI,waist circumference,hip circumference,and WHR in the treatment group were lower than those in the control group(P<0.05).Conclusion:Acupuncture combined with Chine
