“肺瘅”作为社区获得性肺炎中医病名的理论探讨

摘　　要：社区获得性肺炎是临床上常见的感染性疾病,发病率高,严重威胁人类健康。中医药治疗社区获得性肺炎具有独特的优势,但其中医病名繁杂如“风温肺热病”“咳嗽”“喘证”“肺痈”等,目前尚未达成共识。新近发布的《中医临床诊疗术语》明确提出“肺瘅”是以咳嗽,痰黄而稠,或痰中夹血,口渴,可伴见发热,胸胁肋痛,大便秘结,小便黄等为特征的肺系病。笔者进一步求索古籍和文献,根据社区获得性肺炎的临床表现、病因病机,并结合临床实践,确立“肺瘅”作为社区获得性肺炎的中医病名诊断,既能反映其历史沿革、中医病机、疾病特征及治疗大法,更利于中医药对社区获得性肺炎的深入研究。Community-acquired pneumonia is a common infectious disease in clinical practice,with a high incidence and a serious threat to human health.Traditional Chinese medicine has unique advantages in the treatment of community-acquired pneumonia.Although there are many traditional Chinese medicine disease names of this disease,such as“wind-warmth disease of lung-heat”,“cough”,“asthma syndrome”,“pulmonary abscess”,it has not reached a consensus on the name.The newly released Traditional Chinese Medicine Clinical Diagnosis and Treatment Terms clearly states that“Fei Dan”is a pulmonary disease characterized by cough,yellow and thick sputum,or blood in sputum,thirst,accompanied by fever,chest pain,constipation,and yellow urine.The author further sought ancient books and literatures,according to the clinical manifestations,etiology and pathogenesis of community-acquired pneumonia,and combined with clinical practice,to establish“Fei Dan”as the traditional Chinese medicine disease name of community-acquired pneumonia,which could reflect its history,characteristics of traditional Chinese medicine pathogenesis of disease and treatment,and it’s more conducive to in-depth study of traditional Chinese medicine for community-acquired pneumonia.
