玉屏风散配四物汤加减治疗白癜风(肝肾亏虚型)疗效观察

摘　　要：目的分析玉屏风散配四物汤加减治疗白癜风(肝肾亏虚型)临床疗效。方法选取2022年2月—2024年2月襄阳市中心医院皮肤科所收治的肝肾亏虚型白癜风患者212例为研究对象,随机数字表法分为两组,106例对照组采用常规抗白癜风治疗,106例观察组在上述基础上予以玉屏风散配四物汤加减治疗,对比两组治疗效果、中医证候积分、病变区域面积、白癜风严重程度评分指数(VitiligoAreaScoringIndex,VASI)及炎症因子与黑素细胞刺激素的变化情况。结果观察组治疗总有效率为95.28%,远高于对照组的84.91%,组间比较差异显著统计学意义(P<0.05);治疗后观察组中医症状积分低于对照组,且治疗后1个月、2个月、3个月观察组皮肤病变区域面积、VASI评分均低于对照组(P<0.05);治疗后观察组白细胞介素17(Interleukin-17,IL-17)、细胞黏附分子(IntercellularAdhesionMolecule1,ICAM-1)低于对照组,α黑素细胞刺激激素(Alpha-MelanocyteStimulatingHormone,α-MSH)高于对照组(P<0.05)。结论对肝肾亏虚型白癜风患者应用玉屏风散配四物汤加减治疗效果明确,能有效缓解患者症状,缩小白斑面积,抑制炎性介质及黑素细胞刺激素的释放,达到维持皮肤健康的目的,具有较高应用价值。Objective To analyze the clinical efficacy of Yupingfeng Powder(玉屏风散)with Siwu Decoction(四物汤)in the treatment of vitiligo(liver and kidney deficiency type).Methods A total of 212 patients with vitiligo of liver and kidney deficiency type admitted to the Department of Dermatology of Xiangyang Center Hospital from February 2022 to February 2024 were selected as the study subjects,and were divided into two groups by randomized numerical table method:106 cases of the control group were treated with conventional anti-vitiligo treatment,and 106 cases of the observation group were treated with Yupingfeng Powder and Sizi Decoction based on the above mentioned basis,and the treatment effects of the two groups were compared.It compared the effect of the two groups,traditional Chinese medicine symptom scores,lesion area,vitiligo severity scoring index(VASI)and changes in inflammatory factors and melanocyte stimulating hormone.Results The total effective rate in the observation group was 95.28%,which was much higher than that(84.91%)in the control group,and the difference between the groups was significant(P<005).The traditional Chinese medicine symptom score of the observation group was lower than that of the control group after treatment,and the area of the skin lesion area and the VASI score of the observation group were lower than those of the control group at 1,2 and 3 months after treatment(P<005).The level of interleukin-17(IL-17)of the observation group was lower than that of the control group after treatment.The level of intercellular cell adhesion molecule-1(ICAM-1)was lower than that of the control group,and the level ofα-melanocyte-stimulating hormone(α-MSH)was higher than that of the control group(P<005).Conclusion The treatment of Yupingfeng Powder with Siwu Decoction to vitiligo of liver and kidney deficiency type has clear effect,which can effectively alleviate the symptoms of the patients,reduce the area of white patches,inhibit the release of inflammatory mediators and melanocyte-stimulating
