血府逐瘀方结合踝关节定点挤压调整手法及针灸治疗急性踝关节扭伤回顾性分析

摘　　要：目的观察血府逐瘀方结合踝关节定点挤压调整手法及针灸治疗急性踝关节扭伤临床效果。方法回顾性分析2022年1月—2023年6月收治的90例急性踝关节扭伤患者临床资料,根据治疗方法不同将患者分为两组,对照组患者采取常规疗法,观察组患者采取常规疗法结合血府逐瘀方、踝关节定点挤压调整手法及针灸,记录并比较患者临床疗效、肿胀缓解时间与静息疼痛缓解时间、功能恢复时间、患者中医证候总积分与疼痛评分(visual analogue scale,VAS)变化、踝关节Kofoed评分变化与踝关节跖屈活动度及背伸活动度变化、C反应蛋白(C-reactiveprotein,CRP)与白介素-6(interleukin-6,IL-6)、肿瘤坏死因子α(tumour necrosis factor-α,TNF-α)变化及不良反应发生情况。结果观察组患者疗效优于对照组,P<0.05;较对照组,观察组患者肿胀缓解时间与静息疼痛缓解时间、功能恢复时间均更短,P<0.05;治疗后两组患者中医证候总积分、VAS评分、踝关节Kofoed评分、踝关节跖屈活动度及背伸活动度、炎症因子水平(CRP及IL-6、TNF-α)等指标均改善,而观察组患者治疗后中医证候总积分、VAS评分、踝关节Kofoed评分、踝关节跖屈活动度及背伸活动度、炎症因子水平(CRP及IL-6、TNF-α)等指标均比对照组更佳,P<0.05;对照组患者未见不良反应,观察组出现1例口干,P>0.05。结论急性踝关节扭伤患者采取血府逐瘀方结合踝关节定点挤压调整手法及针灸治疗效果显著,患者症状改善,疼痛缓解,踝关节功能恢复好,且恢复快,不良反应少,安全可靠。Objective To observe the clinical effect of Xuefu Zhuyu Recipe(血府逐瘀方)combined with ankle joint point squeeze adjustment technique and acupuncture on acute ankle sprain.Methods The clinical data of 90 patients with acute ankle sprain admitted from January 2022 to June 2023 was retrospectively analyzed.The patients were divided into two groups according to different treatment methods.The patients in the control group received conventional therapy,while those in the observation group received conventional therapy combined with Xuefu Zhuyu Recipe,ankle point compression adjustment manipulation and acupuncture.The clinical efficacy,swelling relief time and resting pain relief time,functional recovery time,the changes of TCM syndrome score and pain scale(VAS),the changes of ankle Kofoed score and ankle plantar flexor and dorsoextension motion,the levels of C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and adverse reactions were re-corded and compared.Results The curative effect of the observation group was better than that of the control group(P<0.05).Compared with those of the control group,the duration of swelling relief,resting pain relief and functional recovery in the observa-tion group were shorter(P<0.05).After treatment,the total scores of TCM symptoms,VAS score,ankle Kofoed score,ankle plantar flexion and dorsoextension motion,and the levels of inflammatory factors(CRP,IL-6,TNF-α)were improved in both groups.After treatment,the total scores of TCM symptoms,VAS score,ankle Kofoed score,ankle plantar flexion and dorsoexten-sion motion,inflammatory factor levels(CRP,IL-6,TNF-α)in the observation group were better than those in the control group(P<0.05).There was no adverse reaction in the control group,and 1 case of dry mouth in the observation group(P>0.05).Conclusion The patients with acute ankle sprain were treated with Xuefu Zhuyu Recipe(血府逐瘀方)combined with an-kle joint point squeeze adjustment technique and acupuncture,and the treatment effect was significa
