散瘀祛痛膏外敷治疗早中期膝骨关节炎疼痛及炎症因子水平的影响

摘　　要：目的探讨散瘀祛痛膏外敷治疗早中期膝骨关节炎疼痛的效果及对血清炎性致痛因子水平的影响。方法收集2023年8月至2024年6月就诊于河南中医药大学第一附属医院骨伤诊疗中心的60例气滞血瘀型早中期膝骨关节炎患者。采用随机数字表法将其分为试验组和对照组,各30例。试验组外敷散瘀祛痛膏,对照组外敷通络祛痛膏,两组疗程均为14 d。收集治疗前后视觉模拟评分法(VAS)评分、奎森功能演算指数(Lequesne)。采用酶联免疫吸附试验法检测血清中致痛因子前列腺素E_(2)(PGE_(2))、前列腺素2α(PGF2α)、5-羟色胺(5-HT)和P物质(SP)。结果治疗后,两组VAS评分、Lequesne评分、PGE_(2)、PGF2α、5-HT、SP水平均较治疗前降低,且试验组低于对照组(P<0.05)。结论散瘀祛痛膏外敷可明显缓解膝骨关节炎疼痛、改善关节功能、降低相关致痛因子,且安全性良好,考虑作用机制可能与降低血清炎性致痛因子水平相关。Objective To explore the therapeutic efficacy of applying Sanyu Qutong Ointment for the treatment of pain in early and middle period of knee osteoarthritis and its impact on the levels of serum inflammatory pain factors.Methods Sixty patients with qi stagnation and blood stasis type of early and mvddle period knee osteoarthritis who visited the First Affiliated Hospital of Henan University of Chinese Medicine from August 2023 to June 2024 were recruited.They were divided into experimental group and control group by random number table method,with 30 cases in each group.The experimental group applied Sanyu Qutong Ointment while the control group applied Tongluo Qutong Ointment.Both groups underwent a treatment course of 14 days.Visual analogue scale(VAS)score and Lequesne functional quotient(Lequesne)were collected before and after treatment,and the serum pain inducing factors prostaglandin E_(2)(PGE_(2))and prostaglandin 2α(PGF2α),5-hydroxytryptamine(5-HT),and substance P(SP)were detected by enzyme-linked immunosorbed assay.Results After treatment,VAS score,Lequesne score,PGE_(2),PGF2α,5-HT,and SP levels in both groups were lower than before treatment,and the experimental group was lower than the control group(P<0.05).Conclusion The application of Sanyu Qutong Ointment can significantly alleviate the pain of knee osteoarthritis,improve joint function,and reduce related pain-causing factors,the security is good,and the mechanism of action is likely related to the reduction of pain-causing factor levels.
