以脑栓通胶囊为例探究中成药临床应用专家共识制定的文献预检索

摘　　要：目的充分利用现有循证医学证据,系统梳理脑栓通胶囊及其同类中成药的文献,以精准定位于临床实践,并构建合理规范的专家共识。方法《脑栓通胶囊临床应用专家共识》已在中华中医药学会标准化办公室申请立项,工作组在标准流程引导下,采用“文献预检索”法,文献方面,检索中国知网、万方数据知识服务平台、维普网和Web of Science核心合集数据库;指南检索世界卫生组织、美国国立指南库、英国国家医疗保健优化研究所、加拿大医学会临床实践指南库、全国标准信息公共服务平台等,检索时间为建库至2024年7月。通过对脑栓通胶囊进行CiteSpace可视化分析,总结脑栓通胶囊的研究热点、同类中成药进入指南或专家共识的情况及相关疾病指南中推荐的用药方案,并针对脑栓通胶囊及相关中成药构建临床问题。结果脑栓通胶囊所涉及的文献数量相对较少,与同类中成药比较,其在文献发表方面的优势较为不足;为解决临床医师所关注的用药问题,建议进行权威专家访谈和大规模的临床调查,以获取专家在药物使用方面的经验,填补现有证据的缺失。结论对脑栓通胶囊文献预检索在构建临床问题中的应用和实践做重点介绍,旨在为中成药专家制定共识积累经验。Objective To make full use of the existing evidence of evidence-based medicine,systematically comb literature of Naoshuantong Capsules and its similar traditional Chinese patent medicines and simple preparations,to accurately locate it in clinical practice,and provide guidance for building expert consensus of reasonable norms.Methods The Expert Consensus on Clinical Application of Naoshuantong Capsules had been applied for approval in Chinese Association of Chinese Medicine.Under the guidance of standard procedures,“literature pre-search”method was used to search literature on CNKI,Wanfang data,VIP,and Web of Science Core Collection.In terms of guidelines,searches were conducted on World Health Organization,National Guideline Clearinghouse,National Institute for Health and care excellence,Canadian Medical Association:clinical practice guideline,and national public service platform for standards information.All databases were searched from the time of their establishment until July 2024.Naoshuantong Capsules was visual analyzed by CiteSpace,research hotspots of Naoshuantong Capsules,the situation of similar traditional Chinese patent medicines and simple preparations entering the guidelines or expert consensus,and recommended medication in the guidelines for related diseases were summarized.Clinical questions about Naoshuantong Capsules and related traditional Chinese patent medicines and simple preparations were raised.Results The number of literatures involved in Naoshuantong Capsules was relatively small.Compared with the similar traditional Chinese patent medicines and simple preparations,Naoshuantong Capsules had insufficient advantages in literature publication.In order to solve the problems of medication use that clinicians were concerned about,authoritative expert interviews and large-scale clinical surveys were recommended to obtain expert experience in drug use,and fill the gaps in existing evidence.Conclusion This paper focuses on application and practice of Naoshuantong capsules literature pre-sear
