免疫功能正常的耶氏肺孢子菌肺炎1例

摘　　要：本文回顾1例免疫功能正常,通过肺泡灌洗液靶向高通量定量测序(tNGS)确诊为耶氏肺孢子菌感染的临床病例。患者既往有反复咳嗽、气喘病史,此次因症状再发加重1个月余于湖北省天门市第一人民医院就诊,入院后经抗感染、抗病毒、平喘等对症治疗后症状未见明显好转,后完善支气管镜检查并留取肺泡灌洗液行tNGS检测,检出非典型病原体耶氏肺孢子菌序列数651,考虑耶氏肺孢子菌肺炎可能,遂调整治疗方案予以卡泊芬净及复方磺胺甲噁唑片抗真菌治疗,经治疗后症状好转,病灶较前减少。本研究针对该病例进行总结分析,旨在提高临床医师对免疫功能正常耶氏肺孢子菌肺炎的认识。The paper reviews a clinical case of Pneumocystis jirovecii infection diagnosed by targeted next-generation sequencing(tNGS)of alveolar lavage fluid with normal immune function.The patient had a history of recurrent coughing and asthma,and was admitted to the First People’s Hospital of Tianmen in Hubei Province,for more than one month due to the worsening symptoms.After admission,the symptoms did not improve significantly after symptomatic treatment such as anti-infection,anti-virus,and anti-asthmatic treatment.After completing bronchoscopy and taking alveolar lavage fluid for tNGS detection,the atypical pathogen Pneumocystis jirovecii sequence number 651 was detected,and the possibility of Pneumocystis jirovecii pneumonia was considered,the treatment regimen was adjusted and antifungal treatment was given to Carpofungine and Trimethoprim-Sulfamethoxazole.After treatment,the symptoms were improved,and the lesions decreased.The purpose of this study is to improve the knowledge of clinicians on understanding of the patients with normal immune function of Pneumocystis jirovecii pneumonia.
