604例自然流产患者体质与影响因素相关性分析

摘　　要：[目的]分析604例自然流产患者的中医体质分布,探讨影响体质的相关因素,为自然流产的防治提供新思路。[方法]采用回顾性研究的方法,收集604例自然流产患者西医病因资料及其根据《昆仑-炎黄健康辨识表》内容进行的中医体质辨析结果,分析自然流产次数、年龄、西医病因与中医体质的相关性。[结果]604例自然流产患者,年龄≥35岁对流产次数有影响[比值比(odds ratio,OR)=1.299,P=0.001]。604例自然流产患者,偏颇体质占89.74%,其中单一偏颇体质患者为26.16%,兼夹两种偏颇体质患者为27.15%,兼夹三种偏颇体质患者为36.42%。气郁质与流产次数有关(P=0.000)。阳虚质(P=0.000)、阴虚质(P=0.000)、湿热质(P=0.002)、血瘀质(P=0.007)及痰湿质(P=0.029)与年龄有关。604例自然流产患者解剖病因与气郁质有关(P=0.016),感染病因与湿热质(P=0.033)和痰湿质(P=0.033)有关,血栓病因与气郁质(P=0.000)、血瘀质(P=0.002)和痰湿质(P=0.000)有关,内分泌病因与痰湿质(P=0.001)和特禀质(P=0.026)有关,免疫学病因与阳虚质(P=0.049)、阴虚质(P=0.005)和血瘀质(P=0.001)有关,遗传病因与各体质均无关(P>0.05)。[结论]年龄≥35岁可增加自然流产发生的风险。阳虚质、气郁质、阴虚质、气虚质是自然流产的易感体质。不同流产次数的自然流产患者偏颇体质存在相似的聚集现象,其中气郁质与流产次数有关。年龄≥35岁自然流产患者阳虚质、阴虚质和血瘀质的占比增加,湿热质和痰湿质的占比减少。自然流产患者解剖病因与气郁质有关,感染病因与湿热质和痰湿质有关,易栓病因与气郁质、血瘀质和痰湿质有关,内分泌病因与痰湿质和特禀质有关,免疫病因与阳虚质、阴虚质和血瘀质有关。[Objective]To analyse the distribution of TCM constitution of spontaneous abortion 604 cases and explore the relevant factors affecting physique,so as to provide new ideas for the prevention and treatment of spontaneous abortion.[Methods]The TCM physique results of 604 patients with spontaneous abortion were collected,and the effects of abortion times,age and etiology on physique were analyzed.[Results]The influence of age≥35 on the number of spontaneous abortion was statistically significant[odds ratio(OR)=1.299,P=0.001].The biased constitution of 604 patients with spontaneous abortion accounted for 89.74%,among them,26.16%were patients with single biased constitution,27.15%with two biased constitution and 36.42%with three biased constitution.According to the comparison of various types of physique between the two groups,the quality of Qi depression was related to the number of miscarriages(P=0.000).According to the comparison of various types of physique between the two groups,the quality of Yang deficiency(P=0.000),Yin deficiency(P=0.000),dampness-heat(P=0.002),blood stasis(P=0.007)and phlegm-dampness(P=0.029)were related to age.The anatomical etiology of spontaneous abortion was related to the quality of Qi depression(P=0.016),the cause of infection was related to the quality of dampness-heat(P=0.033)and phlegm-dampness(P=0.033),the etiology of thrombosis was related to the quality of Qi depression(P=0.000),blood stasis(P=0.002)and phlegm-dampness(P=0.000),and the endocrine etiology was related to phlegm-dampness(P=0.001)and characteristics(P=0.026).The immune etiology was related to Yang deficiency(P=0.049),Yin deficiency(P=0.005)and blood stasis(P=0.001),but the genetic etiology had nothing to do with each constitution(P>0.05).[Conclusion]Age≥35 years old could increase the risk of spontaneous abortion.Yang deficiency,Qi depression,Yin deficiency and Qi deficiency were the susceptible constitutions of spontaneous abortion.There was a similar aggregation phenomenon in the biased constitution of patients
