经前外侧入路与后外侧入路治疗旋后外旋型Ⅳ度踝关节骨折的临床安全性比较

摘　　要：目的比较旋后外旋型Ⅳ度踝关节骨折采用经前外侧入路和后外侧入路治疗的安全性。方法选取旋后外旋型Ⅳ度踝关节骨折患者60例作为本次研究对象,按随机数表法将患者分为对照组(n=30)和研究组(n=30)。对照组采用经后外侧入路治疗,研究组采用经前外侧入路治疗,术后两组患者均进行护理。比较两组患者手术时间、术中出血量、住院时间、并发症发生情况。结果研究组手术时间为(85.72±5.47)min、住院时间为(6.64±1.31)d,均短于对照组的(112.71±9.83)min、(8.49±1.52)d,术中出血量为(106.83±12.59)ml,少于对照组的(146.72±13.26)ml,统计学有意义(P<0.05)。研究组并发症发生率为3.33%,远低于对照组的23.33%,统计学有意义(P<0.05)。结论相较于经后外侧入路,采用经前外侧入路治疗旋后外旋型Ⅳ度踝关节骨折,患者各项手术指标更优,并发症发生率低,治疗效果显著。Objective To compare the safety of anterolateral versus posterolateral approach for the treatment of supination external rotation stageⅣankle fractures.Methods 60 patients with supination external rotation stageⅣankle fractures were selected as the subjects of this study,and were divided into a control group(n=30)and a study group(n=30)according to random numerical table.The control group was treated with posterolateral approach,and the study group was treated with anterolateral approach.Postoperative care was given to both groups.The operation time,intraoperative blood loss,length of hospital stay and complications were compared between the two groups.Results The operation time and length of hospital stay in the study group were(85.72±5.47)min and(6.64±1.31)d,both of which were shorter than(112.71±9.83)min and(8.49±1.52)d in the control group;the intraoperative blood loss of(106.83±12.59)ml in the study group was less than(146.72±13.26)ml in the control group;there was statistical significance(P<0.05).The incidence of complications in the study group was 3.33%,which was significantly lower than 23.33%in the control group(P<0.05).Conclusion Compared with the posterolateral approach,the use of the anterolateral approach for the treatment of supination external rotation stageⅣankle fractures has better surgical indicators,lower complication rate and significant therapeutic effect.
