基于电子健康卡的医联体服务平台构建与应用

摘　　要：为进一步落实分级诊疗制度,扎实推进资源共享、业务协同,实现区域医联体内的居民健康档案管理、分级诊疗等业务的信息共享,本研究结合区域医联体发展的实际情况,依托市卫健委卫生专网,引入分布式集群与开放理念,以电子健康卡作为唯一的身份认证标识,与区域医联体健康平台进行集中对接,搭建了基于电子健康卡的区域医联体健康服务平台。该平台实现了双向转诊渠道来院就诊2689人,下转至医联体单位共3829人次,覆盖病种50余种,开展慢病管理2000余人次,切实改善了患者的就医体验。通过整合线上线下资源,创新优化就诊流程,为区域医联体各级医疗机构的患者提供了预约诊疗、转诊服务、药事管理、健康管理等一体化服务。In order to further implement the hierarchical diagnosis and treatment system,solidly promote resource sharing and business collaboration,and realize the information sharing of residents’health record management and hierarchical diagnosis and treatment within the regional medical alliance,this study combined with the actual situation of the development of the regional medical alliance,relying on the health network of the Municipal Health Commission,introducing the concept of distributed clustering and openness,using the electronic health card as the only identity authentication identifier,and centrally docking with the regional medical alliance health platform,and building a regional medical alliance health service platform based on the electronic health card.The platform realized 2,689 visits to the hospital through two-way referral channels,and a total of 3,829 visits down to the medical consortium units,covering more than 50 kinds of diseases and carrying out chronic disease management for more than 2,000 visits,which effectively improved the patients’medical experience.Through the integration of online and offline resources,innovation and optimization of the consultation process,patients at all levels of medical institutions in the regional medical consortium are provided with integrated services such as appointment and treatment,referral services,pharmacy management and health management.
