INTERTAN髓内钉内固定与人工关节置换术治疗髋部骨折的效果分析

摘　　要：目的探讨INTERTAN髓内钉内固定与人工关节置换治疗髋部骨折的治疗效果。方法随机选取2022年1月—2023年12月关岭自治县人民医院收治的100例髋部骨折患者为研究对象,根据不同治疗方法分为两组,各50例。对照组行INTERTAN髓内钉内固定手术,观察组行人工关节置换术。比较两组患者的手术相关临床指标、骨代谢相关指标及并发症。结果观察组负重时间、住院时间分别为(10.36±1.27)d、(13.98±3.59)d,均短于对照组的(29.78±5.64)d、(21.58±6.67)d,差异有统计学意义(t=23.753、7.095,P均<0.05)。两组手术时间、术后引流量、并发症发生率以及术前骨代谢相关指标结果比较,差异无统计学意义(P均>0.05)。术后,两组患者的骨碱性磷酸酶,甲状旁腺激素与骨γ-羧基谷氨酸蛋白水平均较术前升高,且观察组更高,差异有统计学意义(P均<0.05)。结论与INTERTAN髓内钉内固定手术相比,髋部骨折患者采用人工关节置换术治疗效果更好,有利于改善骨代谢相关指标,可缩短患者术后负重时间及住院时间。Objective To explore the therapeutic effect of INTERTAN intramedullary nail fixation and artificial joint replacement in the treatment of hip fractures.Methods A total of 100 patients with hip fractures admitted to Guanling Autonomous County People's Hospital from January 2022 to December 2023 were randomly selected as the research objects and divided into two groups according to different treatment methods,with 50 cases in each group.The control group underwent INTERTAN intramedullary nail fixation surgery,and the observation group underwent artificial joint replacement surger.The surgical-related clinical indicators,bone metabolism-related indicators and complications of the two groups were compared.Results The weight-bearing time and hospital stay of the observation group were(10.36±1.27)d and(13.98±3.59)d,respectively,which were shorter than those of the control group(29.78±5.64)d and(21.58±6.67)d,the differences were statistically significant(t=23.753,7.095,both P<0.05).The results of operation time,postoperative drainage volume,rate of complications and preoperative bone metabolism-related indicators were similar between the two groups,the differences were no statistically significant(all P>0.05).After surgery,the levels of bone alkaline phosphatase,parathyroid hormone and bone gamma-carboxyglutamic acid protein in the two groups were all higher than those before surgery,and the observation group was higher,the differences were statistically significant(all P<0.05).Conclusion Compared with INTERTAN intramedullary nail fixation surgery,Patients with hip fractures who undergo artificial joint replacement surgery have better treatment outcomes,which is beneficial to improve bone metabolism-related indicators and can shorten the postoperative weight-bearing time and hospital stay of patients.
