全科理念下动态指导高血压伴冠心病患者服用麝香通心滴丸联合苯磺酸氨氯地平片治疗的疗效分析

摘　　要：目的分析全科理念下动态指导高血压伴冠状动脉粥样硬化性心脏病(coronary heart disease,CHD)患者服用麝香通心滴丸联合苯磺酸氨氯地平片治疗的疗效。方法随机选取2020年10月—2022年10月永安市立医院收治的170例高血压伴CHD患者为研究对象,依据不同治疗方法分为观察组(n=85)与对照组(n=85),观察组采用全科理念下动态指导的麝香通心滴丸联合苯磺酸氨氯地平片治疗、对照组采用苯磺氨氯地平片治疗,两组患者均连续治疗3个月,比较两组患者临床疗效、治疗前后的心功能[每搏输出量(stroke vol-ume,SV)、左心室舒张末期内径(left ventricular end diastolic dimension,LVEDD)、左心室射血分数(left ven-tricular ejection fraction,LVEF)]、血液流变学指标[全血低切黏度、全血高切黏度、纤维蛋白原(fibrinogen,FIB)]水平。结果观察组治疗总有效率为97.65%(83/85),高于对照组的89.41%(76/85),差异有统计学意义(χ2=4.763,P<0.05)。治疗前,两组患者心功能指标、血液流变学指标比较,差异无统计学意义(P均>0.05);治疗后,观察组LVEDD、全血低切及高切黏度、FIB低于对照组,SV、LVEF高于对照组,差异有统计学意义(P均<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论全科理念下动态指导高血压伴CHD患者服用麝香通心滴丸联合苯磺酸氨氯地平片疗效更佳,联合用药能提升患者心功能,改善患者血液流变学指标,用药安全性高。Objective To analyze the therapeutic effect of dynamic guidance of Shexiang Tongxin Diwan combined with amlodipine besylate tablets in the treatment of patients with hypertension and coronary heart disease(CHD)under the general philosophy.Methods A total of 170 patients with hypertension and CHD admitted to Yongan Municipal Hospital from October 2020 to October 2022 were randomly selected as the study objects,and were divided into observation group(n=85)and control group(n=85)according to different treatment methods.Observation group was treated with Shexiang Tongxin dropping pills combined with amlodipine besylate tablets under the dynamic guidance of general practice,and control group was treated with amlodipine besylate tablets.Both groups were treated continuously for 3 months.The clinical efficacy,cardiac function[stroke volume(SV),left ventricular end diastolic dimension(LVEDD),left ventricular ejection fraction(LVEF)]and hemorheology indexes[low viscosity of whole blood,high viscosity of whole blood,fibrinogen(FIB)]before and after treatment were compared between the two groups.Results The effective rate of the observation group was 97.65%(83/85),which was higher than that of the control group 89.41%(76/85),the difference was statistically significant(χ2=4.763,P<0.05).Before treatment,there was no significant dif ference in cardiac function index and hemorheology index between the two groups(both P>0.05);after treatment,LVEDD,whole blood viscosity of low cut and high cut,FIB and SV and LVEF in observation group were lower than those in control group,and the differences were statistically significant(all P<0.05).There was no significant differ ence in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The dynamic guidance of Shexiang Tongxin Diwan combined with amlodipine besylate tablets in patients with hypertension and CHD under the concept of general practice is more effective,and the combination of medication can significantly improve the cardiac function of patients,improve
