摘 要:目的探讨麻醉前2 h口服葡萄糖在择期非胃肠道手术患者中的应用价值。方法回顾性选取2021年10月—2022年12月昆明医科大学第一附属医院行择期非胃肠道手术的200例患者的临床资料,根据术前2 h有无口服葡萄糖分为观察组、对照组,各100例。观察组于麻醉前2 h给予10%葡萄糖,对照组麻醉前2 h未服用任何液体。分析两组主观舒适度、麻醉诱导前胃排空完成率、术后恶心呕吐发生率、麻醉前血糖及胰岛素浓度。结果观察组主观舒适度中口渴感、饥饿感及焦虑感的比例均低于对照组,差异有统计学意义(P均<0.05)。两组麻醉诱导前胃排空完成率比较,差异无统计学意义(P>0.05)。观察组术后恶心呕吐发生率4.00%(4/100),低于对照组的13.00%(13/100),差异有统计学意义(χ2=5.207,P<0.05);观察组血糖、胰岛素浓度水平高于对照组,差异有统计学意义(P均<0.05)。结论麻醉前2 h口服少量葡萄糖,可显著改善非胃肠道手术患者舒适度,减少术后恶心呕吐发生率,提高麻醉前血糖水平及胰岛素浓度。Objective To explore the application value of oral glucose 2 hous before anesthesia in patients undergoing elective non-gastrointestinal surgery.Methods The clinical data of 200 patients who underwent elective non-gastrointestinal surgery in the First Affiliated Hospital of Kunming Medical University from October 2021 to December 2022 were retrospectively selected.According to the presence or absence of oral glucose 2 hour before surgery,they were divided into observation group and control group,with 100 cases in each group.The observation group was given 10%glucose 2 hour before anesthesia,and the control group was not given any liquid 2 hour before anesthesia.The subjective comfort,the completion rate of gastric emptying before anesthesia induction,the incidence of postoperative nausea and vomiting,blood glucose and insulin concentration before anesthesia were analyzed in the two groups.Results The proportion of thirst,hunger and anxiety in the subjective comfort of the observation group were lower than those of the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in the completion rate of gastric emptying between the two groups before anesthesia induction(P>0.05).The incidence of postoperative nausea and vomiting in the observation group was 4.00%(4/100),which was lower than 13.00%(13/100)in the control group,and the difference was statistically significant(χ2=5.207,P<0.05).The levels of blood glucose and insulin in the observation group were higher than those in the control group,and the differences were statistically significant(both P<0.05).Conclusion Oral administration of a small amount of glucose 2 hour before anesthesia can significantly improve the comfort of patients undergoing non-gastrointestinal surgery,reduce the incidence of postoperative nausea and vomiting,and increase the blood glucose level and insulin concentration before anesthesia.
