摘　　要：目的观察在对输血治疗过程中开展Rh血型系统抗原检测对受血者机体免疫力以及不良反应的影响。方法以2023年4月至2024年5月中国人民解放军联勤保障部队第九一〇医院输血治疗患者120例为研究对象,分析各方面临床资料,其中60例(观察组)受-供体E抗原均为阴性,另60例(对照组)受血者E抗原阴性,供血者E抗原为阳性。针对两组患者输血治疗期间免疫球蛋白水平、T淋巴细胞亚群、不良反应进行统计。结果输血前,两组IgM、IgG水平比较,差异无统计学意义(P>0.05);输血后,观察组IgM、IgG水平较对照组低(P<0.05)。观察组输血后不良反应发生率低于对照组(P<0.05)。针对T淋巴细胞亚群统计,输血前两组差异无统计学意义(P>0.05),输血后观察组CD4+低于对照组,CD8+高于对照组(P<0.05)。针对白细胞介素-2水平对比,输血前两组差异无统计学意义(P>0.05),输血后观察组低于对照组(P<0.05)。结论在进行输血治疗前开展Rh血型系统抗原检测,有助于实现精准输血,可以减少输血治疗对患者免疫水平造成的影响,并对输血不良反应进行预防。Objective To observe the influence of Rh blood group system antigen detection on the immunity and adverse reactions of blood recipients in the course of transfusion treatment.Methods From April 2023 to May 2024,120 patients receiving blood transfusion treatment in the 910th Hospital of the Joint Service Support Force of the Chinese People’s Liberation Army were selected as the research objects.Clinical data were analyzed in all aspects.Among them,60 patients(observation group)were negative for E-antigen of recipients and donors,while 60 patients(control group)were negative for E-antigen of recipients and positive for E-antigen of donors.The immunoglobulin level,T lymphocyte subsets and adverse reactions of patients in the two groups were analyzed.Results Before blood transfusion,there was no significant difference in IgM and IgG levels between the two groups(P>0.05).After transfusion,the levels of IgM and IgG in the observation group were lower than those in the control group(P<0.05).The incidence of adverse reactions after transfusion was lower in the observation group than in the control group(P<0.05).There was no significant difference in T lymphocyte subsets between the two groups before transfusion(P>0.05).After transfusion,CD4+in observation group was lower than that in control group,and CD8+was higher than that in control group(P<0.05).There was no significant difference in interleukin-2 level between the two groups before blood transfusion(P>0.05).After transfusion,the observation group was lower than the control group(P<0.05).Conclusions Rh blood group system antigen detection before transfusion is helpful to achieve accurate blood transfusion,reduce the impact of transfusion treatment on patients’immune level,and prevent transfusion adverse reactions.
