二氧化碳点阵激光联合富血小板血浆治疗萎缩性痤疮瘢痕的临床疗效

摘　　要：目的分析二氧化碳点阵激光联合富血小板血浆治疗萎缩性痤疮瘢痕的疗效。方法随机选取2022年7月—2024年3月泉州市皮肤病防治院收治的130例萎缩性痤疮瘢痕患者为研究对象,依据不同治疗方案分为两组,各65例。对照组二氧化碳点阵激光治疗,研究组联合富血小板血浆治疗,对比观察临床疗效、瘢痕情况[温哥华瘢痕量表(Vancouver Scar Scale,VSS)]、不良事件(水肿、红斑、瘙痒、色素沉着)发生率、临床症状(皮肤潮红、皮肤红斑、丘疹脓疱、毛细血管扩张)评分。结果研究组临床疗效优于对照组,差异有统计学意义(P<0.05)。治疗后,研究组瘢痕情况评分中色泽评分为(1.18±0.28)分,厚度评分为(1.24±0.13)分,血管分布评分为(1.07±0.86)分,柔软度评分为(1.23±0.37)分,均低于对照组的(1.69±0.31)分、(1.67±0.36)分、(1.56±0.97)分、(1.85±0.46)分,差异有统计学意义(t=9.843、9.057、3.047、8.467,P均<0.05)。两组患者不良事件总发生率对比,差异无统计学意义(P>0.05)。治疗前,两组患者临床症状评分对比,差异无统计学意义(P>0.05);治疗后,研究组临床症状评分低于对照组,差异有统计学意义(P<0.05)。结论萎缩性痤疮瘢痕患者采用二氧化碳点阵激光联合富血小板血浆治疗具有较高疗效和安全性,能够促进临床症状缓解。Objective To analyze the effect of carbon dioxide dot laser combined with platelet rich plasma in the treatment of atrophic acne scar.Methods A total of 130 patients with atrophic acne scar admitted to Quanzhou Dermatology Prevention and Treatment Hospital from July 2022 to March 2024 were randomly selected as the study subjects and divided into two groups with 65 cases each according to different treatment regimens.The control group was treated with carbon dioxide dot matrix laser,and the study group was treated with platelet-rich plasma.Clinical efficacy,Scar status[Vancouver Scar Scale(VSS)],incidence of adverse events(edema,erythema,pruritus,pigmentation),clinical symptoms(skin flushing,skin erythema,papular pustules,telangiectasia)score were compared and observed.Results The clinical effect of the study group was better than that of the control group,the difference was statistically significant(P<0.05).After treatment,the scar color score of the study group was(1.18±0.28)points,the thickness score was(1.24±0.13)points,the vascular distribution score was(1.07±0.86)points,and the flexibility score was(1.23±0.37)points,they were all lower than(1.69±0.31)points,(1.67±0.36)points,(1.56±0.97)points and(1.85±0.46)points in the control group,and the differences were statistically significant(t=9.843,9.057,3.047,8.467,all P<0.05).There was no significant difference in the total incidence of adverse events between the two groups(P>0.05).Before treatment,there was no significant difference in clinical symptom scores between the two groups(P>0.05);after treatment,the clinical symptom score of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of patients with atrophic acne scar with carbon dioxide dot matrix laser combined with platelet-rich plasma has high efficacy and safety,and can promote the relief of clinical symptoms.
