摘  要:目的应用近红外功能成像技术(fRINS)分析中、重度脊柱慢性疼痛患者治疗前后大脑前额叶皮层氧合血红蛋白水平,为评价治疗效果及预后判断提供量化理论依据。方法选取江西省人民医院康复医学科2022年1月至2022年6月收治的30例脊柱慢性疼痛患者,依照视觉模拟疼痛评分(VAS)分为中度疼痛组(14例)、重度疼痛组(16例)。通过fRINS检测前额叶皮层氧合血红蛋白水平,经2周的临床治疗(依托考昔片、超短波物理因子、手法)后再次评估VAS评分,并通过fRINS检测前额叶皮层氧合血红蛋白水平。采用SPSS 23.0分析数据。结果治疗后两组VAS评分及前额叶皮层氧合血红蛋白水平较治疗前均降低(P<0.05)。结论fRINS可将疼痛程度进行量化,其可成为一种简单易得、无创、客观的疼痛评估指标,为评估临床镇痛疗效及预后判断提供客观数据,为慢性疼痛的治疗提供理论基础。Objective Near-infrared functional imaging(fRINS)was used to analyze the changes of oxyhemoglobin concentration in the prefrontal cortex of patients with moderate and severe chronic spinal pain before and after treatment,so as to provide a quantitative theoretical basis for evaluating the therapeutic effect and prognosis.Methods From January 2022 to June 2022,a total of 30 patients with chronic spinal pain in the Rehabilitation Medicine Department of Jiangxi Provincial People’s Hospital were selected and divided into moderate pain group(14 cases)and severe pain group(16 cases)according to visual analogue score(VAS).The concentration of cerebral oxygenated hemoglobin in the prefrontal cortex was detected by fRINS.After 2 weeks of clinical treatment(etoricoxib tablets,ultrashort wave short-wave physical factors,manipulation),the VAS score was evaluated and the concentration of cerebral oxygenated hemoglobin in the prefrontal cortex were detected by fRINS.SPSS 23 software was used for data analysis.Results After treatment,both groups showed a decrease in VAS scores and levels of oxygenated hemoglobin in the prefrontal cortex compared to before treatment(P<0.05).Conclusion fRINS can quantify the degree of pain,which can be used as a simple,non-invasive and objective pain assessment index.It provides objective data for evaluating clinical analgesic efficacy and prognosis,and provides a theoretical basis for the treatment of chronic pain.
