基于临床路径的围手术期整体护理服务在肛瘘患者中的应用效果

摘　　要：目的研究基于临床路径的围手术期整体护理服务对肛瘘患者术后疼痛程度及预防尿潴留的效果。方法选取2020年1月至2023年1月在天津市人民医院接受手术治疗的64例肛瘘患者,随机分为两组。对照组32例接受常规护理,观察组32例接受基于临床路径的围手术期整体护理,观察两组术后疼痛程度、排尿情况以及自我护理技能情况。结果两组患者术后视觉模拟评分法(VAS)评分差异有统计学意义(P<0.05),并且观察组评分低于对照组(P<0.05)。观察组术后首次排尿时间短于对照组,尿潴留和尿路刺激征发生率低于对照组(P<0.05)。观察组自护能力量表(ESCA)评分高于对照组(P<0.05)。结论基于临床路径的围手术期整体护理可以缓解肛瘘患者术后疼痛,降低尿潴留发生率。Objective To study the effect of perioperative holistic nursing service based on clinical pathway on postoperative pain and urinary retention in patients with anal fistula.Methods A total of 64 patients with anal fistula who underwent surgical treatment at Tianjin People’s Hospital from January 2020 to January 2023 were selected as the research subjects and randomly divided into two groups using a random number table method.The control group(32 cases)received routine care,while the observation group(32 cases)received perioperative holistic care based on clinical pathways.The postoperative pain level,urination status,and self-care skills of both groups were observed.Results There was statistically difference in postoperative visual analog scale(VAS)scores between the two groups of patients,and the observation group had lower scores(P<0.05).The first urination time in the observation group was shorter,and the incidence of urinary retention and urinary tract irritation was lower(P<0.05).The exercise of self-care agency scale(ESCA)scores of the observation group were higher(P<0.05).Conclusion Perioperative holistic nursing based on clinical pathway could significantly relieve postoperative pain and reduce the incidence of urinary retention in patients with anal fistula.
