基于4R模式的延伸护理服务在原发性肝癌患者肝动脉化疗栓塞术后的应用

摘　　要：目的探讨和评估基于识别需求、协调资源、实施干预、评价调整4R模式的延伸护理服务在原发性肝癌患者肝动脉化疗栓塞术后的效果。方法采用前瞻性随机对照研究设计,选择2021年12月至2022年12月安阳市第五人民医院接收的64例原发性肝癌患者,分为常规组、4R组,各32例。常规组接受一般护理干预,4R组接受包括全面健康评估、多学科团队资源协调、个体化疼痛管理、心理支持以及康复指导等在内的4R延伸护理服务。通过Herth希望量表、焦虑自评量表、视觉模拟量表对患者的心理状态、疼痛程度进行量化评估。结果干预后,4R组Herth希望量表评分高于常规组,焦虑自评量表评分、视觉模拟量表评分、并发症发生率低于常规组,差异有统计学意义(P<0.05)。结论基于4R模式的延伸护理服务,能够有效预防和减少并发症,显著提升患者的希望感,减轻焦虑情绪,控制疼痛效果良好,值得临床推广应用。Objective To explore and evaluate the effect of extended nursing service based on 4R model of recognition,resourcefulness,respect and reassessment in patients with primary liver cancer after transcatheter arterial chemoembolization.Methods A prospective randomized controlled study design was used to select 64 patients with primary liver cancer admitted to Anyang Fifth People’s Hospital from December 2021 to December 2022,and they were divided into routine group and 4R group,with 32 cases in each group.The routine group received general nursing intervention,and the 4R group received 4R extended nursing services including comprehensive health assessment,multi-disciplinary team resource coordination,individualized pain management,psychological support,and rehabilitation guidance.The patients’mental state and pain degree were quantitatively evaluated by Herth hope scale,self-rating anxiety scale and visual analog scale.Results After intervention,the score of Herth hope scale in 4R group was higher than that in routine group,and the score of self-rating anxiety scale,visual analog scale and complication rate were lower than those in routine group(P<0.05).Conclusion The extended nursing service based on 4R model can effectively prevent and reduce complications,significantly improve patients’sense of hope,relieve anxiety and control pain,and is worthy of clinical application.
