摘 要:目的:探讨不同心血管危险分层高血压患者的肠道菌群水平及其与代谢指标的相关性。方法:回顾性选择2019年2月至2022年5月航天中心医院接诊的127例原发性高血压患者,根据心血管危险分层将患者分为低中危组(n=37)、高危组(n=52)及极高危组(n=38),比较三组患者的血脂、血肌酐,肠道菌群水平;采用Spearman法分析肠道菌群水平与血脂、血肌酐水平的相关性。结果:随着心血管危险分层的上升,低中危组、高危组及极高危组双歧杆菌、多形拟杆菌、乳酸杆菌水平依次显著降低,血肌酐、直肠真杆菌、大肠杆菌及肠球菌水平依次显著升高(P均<0.001)。Spearman相关分析显示,高血压患者心血管危险分层与血肌酐、直肠真杆菌、大肠杆菌及肠球菌水平呈显著正相关(r=0.803~0.912,P均<0.001);与双歧杆菌、多形拟杆菌、乳酸杆菌水平呈显著负相关(r=-0.710~-0.834,P均<0.001)。结论:高血压患者不同心血管危险分层与肠道菌群水平之间具有密切联系,说明菌群可能在一定程度上调控血压。Objective:To investigate intestinal flora levels in hypertensive patients with different cardiovascular risk level and their correlation with metabolic indicators.Methods:A total of 127 patients with essential hypertension admitted in Aerospace Central Hospital between February 2019 and May 2022 were retrospectively selected.According to cardiovascular risk stratification,they were divided into low-medium risk group(n=37),high risk group(n=52)and extremely high risk group(n=38).Levels of blood lipids,serum creatinine(Scr)and intestinal flora were compared among three groups;association of intestinal flora levels with blood lipid,Scr levels were analyzed by Spearman method.Results:As cardiovascular risk level increased,levels of Bifidobacterium,Bacteroides thetaiotaomicron and Lactobacillus significantly reduced,and levels of Scr,Eubacterium rectale,Escherichia coli and Enterococcus significantly rose in the order of low-medium risk group,high risk group and extremely high risk group(P<0.001 all).Spearman correlation analysis indicated that cardiovascular risk level was significant positively correlated with levels of Scr,Eubacterium rectale,Escherichia coli and Enterococcus(r=0.803~0.912,P<0.001 all),and significant inversely correlated with levels of Bifidobacterium,Bacteroides thetaiotaomicron and Lactobacillus(r=-0.710~-0.834,P<0.001 all)in hypertensive patients.Conclusion:Cardiovascular risk level is closely associated with intestinal flora levels in hypertensive patients,suggesting that flora may regulate blood pressure to some extent.
