呼吸训练仪和主动循环呼吸技术对早期肺癌患者术后肺功能及生活质量的影响

摘  要:目的探讨早期使用呼吸训练仪和主动循环呼吸技术(ACBT)对早期肺癌患者术后肺功能及生活质量的影响。方法回顾性选取2022年1月至2023年11月海宁市人民医院胸外科收治的行胸腔镜下肺叶切除或部分肺叶切除术的178例早期肺癌患者为研究对象,根据是否使用呼吸训练仪和ACBT将患者分为干预组(使用呼吸训练仪和ACBT)90例和对照组(未使用呼吸训练仪和ACBT)88例。比较两组患者治疗前,术后第4周、第8周、第12周第一秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1占FVC的比值、FEV1占正常预计值、6 min步行试验结果、慢性阻塞性肺疾病评估测试(CAT)评分和健康调查简表各维度评分,治疗前,术后第3天、第5天、第4周、第8周、第12周最大吸气压(PImax)和最大呼气压力值。结果干预组患者术后第4周、第8周、第12周FEV1、FVC、FEV1占正常预计值均高于对照组,术后第3天、第4周、第8周PImax值均高于对照组,差异均有统计学意义(均P<0.05)。干预组患者术后第4周、第8周生理功能、生理职能、活力评分均高于对照组,差异均有统计学意义(均P<0.05)。两组患者术后第4周、第8周、第12周6 min步行试验结果和CAT评分比较差异均无统计学意义(均P>0.05)。结论早期肺癌患者术后尽早使用呼吸训练仪和ACBT,可有效改善患者肺功能,促进康复,提升生活质量,具有较好的应用价值。Objective To analyze the effects of early use of respiratory training devices and active cycle of breathing techniques(ACBT)on postoperative lung function and quality of life in patients with early lung cancer.Methods A total of 178 patients with thoracoscopic lobectomy or partial lobectomy admitted to the Department of Thoracic Surgery of Haining People's Hospital from January 2022 to November 2023 were selected as the study objects.According to whether respiratory training devices and ACBT were used,the objects were divided into the intervention group(90 cases)and the control group(88 cases).The forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),FEV1/FVC,FEV1 percentage to the predicted normal value,6-minute walk test results,chronic obstructive pulmonary disease assessment test(CAT)score,and each dimension score of the MOS 36-Item Short Form Health Survey of the two groups of patients before treatment,and at the 4th,8th,and 12th weeks after the operation were compared.The maximum inspiratory pressure(PImax)and maximum expiratory pressure values before treatment,and at 3rd day,5th day,4th week,8th week,and 12th week after the operation were also compared.Results The FEV1,FVC,and FEV1 percentage to the predicted normal value in the intervention group were higher than those of the control group at the 4th,8th and 12th weeks after surgery,and the PImax value were higher than those of the control group at the 3rd day,4th and 8th weeks after surgery,the differences were statistically significant(all P<0.05).The physiological function,physical role and vitality in the intervention group were significantly higher than those of the control group at the 4th and 8th weeks after surgery(all P<0.05).There were no significant differences in 6-minute walk test and CAT scores between the two groups at the 4th,8th and 12th weeks after surgery(all P>0.05).Conclusion Early use of respiratory training devices and ACBT after surgery in patients with early lung cancer can effectively improve lung function
