摘　　要：目的分析广东省珠海市某三甲医院与社区卫生服务中心联合健康管理模式(医院-社区联合健康管理模式)对糖尿病患者的干预效果,为提高糖尿病患者血糖控制达标率提供依据。方法2023年1月,将珠海市2个社区卫生服务中心随访的316名成人糖尿病患者纳为研究对象,对其开展医院-社区联合健康管理干预,收集研究对象在干预前和干预1年后对糖尿病健康管理的知-信-行评分及血糖等相关监测数据,进行描述性统计分析。结果共有288例糖尿病患者参与了全程研究并收集到完整信息,干预后患者的知-信-行评分(总分100分)中位数(四分位数)[M(P25,P75)]为78.00(72.00,82.00)分,高于干预前的72.00(66.00,77.00)(Z=-9.365,P<0.05);干预后的血糖控制达标率为88.9%(256/288),高于干预前的71.5%(206/288)(χ^(2)=28.115,P<0.05)。结论对糖尿病患者进行医院-社区联合健康管理干预,可提高患者对糖尿病的知-信-行水平及血糖控制达标率。Objective To analyze the intervention outcomes of the integrated health management model between an A-grade tertiary hospital and community health service centers(hospital-community integrated health management model)in Zhuhai City on diabetic patients,with the aim of providing evidence for improving the rate of meeting targets for glycemic control among diabetic patients.Methods In January 2023,316 diabetic patients followed up by two community health service centers in Zhuhai City were enrolled as study subjects for the hospital-community integrated health management intervention.The monitoring data on their knowledge-attitude/belief-practice(KAP)scores and blood sugar testing related to diabetes health management before and one year after the intervention were collected for descriptive statistical analysis.Results A total of 288 diabetic patients participated the whole study,and their complete information were collected.After the intervention,the median(interquartile)[M(P25,P75)]of the KAP score(out of 100)was 78.00(72.00,82.00),which was higher than that before the intervention score of 72.00(66.00,77.00)(Z=-9.365,P<0.05);the rate of meeting targets for glycemic control after the intervention was88.9%(256/288),higher than that before the intervention,which was 71.5%(206/288)(χ^(2)=28.115,P<0.05).Conclusions The hospital-community integrated health management model can improve the level of KAP and the rate of meeting targets for glycemic control among diabetic patients.
