白长川以清胃凉血法为主治疗原发免疫性血小板减少症经验

摘　　要：原发免疫性血小板减少症是一种发病机制复杂的自身免疫病。白长川教授认为其病机为阳明胃经热毒内迫营血,提出清胃凉血的总体治疗原则,发病阶段以清胃散化裁,清胃中之热毒,佐以犀角地黄汤、清营汤,透营凉血、解毒消斑;瘥后防复阶段以自创芪山地骨皮饮化裁,清血中余热,和血养血,同时兼顾调节免疫稳态。并附验案1则。Primary immune thrombocytopenia(ITP)is an autoimmune disorder with a complex pathogenesis.Professor Bai Changchuan believes that the core pathogenesis of the disorder is heat toxicity in the Yangming stomach meridian,which impairs the nutritive blood.Therefore,he proposes the holistic treatment principle of clearing the stomach and cooling the blood.During the acute phase,the formula Qingwei San is adapted to eliminate stomach heat and toxicitiy,supplemented by Xijiao Dihuang Tang and Qingying Tang to penetrate the nutritive level,cool the blood,resolve toxicity,and eliminate purpura.In the post-recovery phase,Professor Bai modifies his self-formulated prescription(Qishan Digupi Yin)to clear residual heat from the blood,harmonize and nourish the blood,while also regulating immune homeostasis.One case is presented as supporting evidence.
