地佐辛联合舒芬太尼在腹腔镜子宫切除术后患者自控静脉镇痛中的应用效果

摘　　要：目的:观察地佐辛联合舒芬太尼在腹腔镜子宫切除术后患者自控静脉镇痛(PCIA)中的应用效果。方法:回顾性分析2022年9月至2023年9月于该院行腹腔镜子宫切除术的84例患者的临床资料,按照镇痛药物不同将其分为对照组和观察组各42例。对照组术后予以舒芬太尼PCIA,观察组在对照组基础上联合地佐辛PCIA。比较两组术后不同时间(术后12、24、48 h)疼痛[视觉模拟评分法(VAS)]、舒适度[舒适度量表(BCS)]评分,镇痛泵有效按压次数,手术前后应激指标[皮质醇(Cor)、去甲肾上腺素(NE)]水平,以及不良反应发生率。结果:观察组术后12、24、48 h VAS评分均低于对照组,差异有统计学意义(P<0.05);观察组术后12、24、48 h BCS评分均高于对照组,差异有统计学意义(P<0.05);观察组镇痛泵有效按压次数少于对照组,差异有统计学意义(P<0.05);术后48 h,两组Cor、NE水平均高于术前,但观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:地佐辛联合舒芬太尼应用于腹腔镜子宫切除术后PCIA中可降低术后疼痛评分和应激指标水平,提高术后舒适度评分,减少镇痛泵有效按压次数,效果优于单纯舒芬太尼PCIA。Objective:To observe application effects of Dezocine combined with Sufentanil in patient-controlled intravenous analgesia(PCIA)after laparoscopic hysterectomy.Methods:The clinical data of 84 patients who underwent laparoscopic hysterectomy in this hospital from September 2022 to September 2023 were retrospectively analyzed.According to the different analgesic drugs,they were divided into control group and observation group,42 cases in each group.The control group was given Sufentanil PCIA after the surgery,while the observation group was combined with Dezocine PCIA on the basis of that of the control group.The scores of pain[visual analogue scale(VAS)]and comfort[Bruggrmann comfort scale(BCS)]at different time points(12,24 and 48 h after the surgery),the effective pressing times of analgesia pump,the stress indexes[cortisol(Cor),norepinephrine(NE)]levels before and after the surgery,and the incidence of adverse reactions were compared between the two groups.Results:The VAS scores of the observation group were lower than those of the control group 12,24 and 48 h after the surgery,and the differences were statistically significant(P<0.05).The BCS scores of the observation group 12,24 and 48 h after the surgery were higher than those of the control group,and the differences were statistically significant(P<0.05).The effective pressing times of analgesic pump in the observation group were less than those in the control group,and the difference was statistically significant(P<0.05).48 h after the surgery,the levels of Cor and NE in the two groups were higher than those before the surgery,but those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Dezocine combined with Sufentanil in PCIA after laparoscopic hysterectomy can reduce the postoperative pain scores and the stress index levels,improve the postoperative comfort
