化瘀理筋针刺疗法联合通络祛痛膏贴敷治疗腰椎间盘突出症患者的效果

摘　　要：目的:观察化瘀理筋针刺疗法联合通络祛痛膏贴敷治疗腰椎间盘突出症患者的效果。方法:选取2021年6月至2023年6月该院收治的90例腰椎间盘突出症患者进行前瞻性研究,按照随机数字表法分为观察组与对照组各45例。对照组采用通络祛痛膏贴敷治疗,观察组在对照组基础上采用化瘀理筋针刺疗法治疗,比较两组治疗总有效率、腰椎日本骨科协会评估治疗(JOA)评分、Oswestry功能障碍指数(ODI)评分、血清疼痛介质(前列腺素、P物质)水平和血清炎性因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)]水平。结果:观察组治疗总有效率为93.33%,明显高于对照组的77.78%,差异有统计学意义(P<0.05);治疗后,两组腰椎JOA评分高于治疗前,且观察组高于对照组,两组ODI评分,前列腺素、P物质等血清疼痛介质水平,以及CRP、IL-6等血清炎性因子水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:化瘀理筋针刺疗法联合通络祛痛膏贴敷治疗腰椎间盘突出症患者可提高治疗总有效率和腰椎JOA评分,以及降低ODI评分、血清疼痛介质水平和炎性因子水平,效果优于单纯通络祛痛膏贴敷治疗。Objective:To observe effects of Huayu Lijin acupuncture therapy combined with Tongluo Qutong ointment in treatment of patients with lumbar disc herniation.Methods:A prospective study was conducted on 90 patients with lumbar disc herniation admitted to this hospital from June 2021 to June 2023.According to the random number table method,they were divided into observation group and control group,45 cases in each group.The control group was treated with Tongluo Qutong ointment,while the observation group was treated with Huayu Lijin acupuncture on the basis of that of the control group.The total effective rate of treatment,the lumbar Japanese Orthopaedic Associationscores(JOA)score,the Oswestry dysfunction index(ODI)score,the serum pain mediators(prostaglandin,substance P)levelss,and the serum inflammatory factors[C-reactive protein(CRP),interleukin-6(IL-6)]level were compared between the two groups.Results:The total effective rate of the observation group was 93.33%,which was significantly higher than 77.78%of the control group,and the difference was statistically significant(P<0.05).After the treatment,the lumbar JOA scores of the two groups were higher than those before the treatment,and that in the observation group was higher than that in the control group;the ODI score and the levels of serum pain mediators such as prostaglandin and substance P,and serum inflammatory factors such as CRP and IL-6 in the two groups were lower than those before the treatment,and those in the observation group were lower than those in the control group;and the differences were statistically significant(P<0.05).Conclusions:Huayu Lijin acupuncture combined with Tongluo Qutong ointment can improve the total effective rate and the lumbar JOA scores,and reduce the ODI scores,serum pain mediator levels and inflammatory factor levels in the patients with lumbar disc herniation.Moreover,it is superior to simple Tongluo Qutong ointment application.
