Angiojet机械血栓清除术与外周静脉溶栓治疗急性下肢深静脉血栓形成患者的效果比较

摘　　要：目的:比较Angiojet机械血栓清除术与外周静脉溶栓治疗急性下肢深静脉血栓形成患者的效果。方法:回顾性分析2020年6月至2023年6月该院收治的60例急性下肢深静脉血栓形成患者的临床资料,依据不同治疗方法将其分为研究组(n=30)与对照组(n=30)。研究组行Angiojet机械血栓清除术治疗,对照组行外周静脉溶栓治疗。比较两组治疗前后患-健侧周径差、下肢静脉通畅程度评分、凝血功能指标[D-二聚体(D-D)、纤维蛋白原(FIB)]水平,以及并发症/不良反应发生率。结果:治疗后1、3、6 d,两组患-健侧周径差均小于治疗前,且研究组小于对照组,差异有统计学意义(P<0.05);治疗后3、6 d,两组下肢静脉通畅程度评分均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);治疗后7 d,两组FIB、D-D水平均低于治疗前,且研究组FIB、D-D水平均低于对照组,差异有统计学意义(P<0.05);两组并发症/不良反应发生率比较,差异无统计学意义(P>0.05)。结论:Angiojet机械血栓清除术治疗急性下肢深静脉血栓形成患者可降低患-健侧周径差、下肢静脉通畅程度评分和凝血功能指标水平的效果优于外周静脉溶栓治疗。Objective:To compare effects of Angiojet mechanical thrombectomy and peripheral venous thrombolysis in treatment of patients with acute lower extremity deep venous thrombosis.Methods:The clinical data of 60 patients with acute lower extremity deep venous thrombosis admitted to this hospital from June 2020 to June 2023 were retrospectively analyzed.According to different treatment methods,they were divided into study group(n=30)and control group(n=30).The study group was treated with Angiojet mechanical thrombectomy,while the control group was treated with peripheral intravenous thrombolysis.The levels of circumference difference between the affected and healthy sides,the lower limb venous patency score,the levels of coagulation function indexes[D-dimer(D-D),fibrinogen(FIB)]before and after the treatment,and the incidence of complications/adverse reactions were compared between the two groups.Results:At 1,3 and 6 days after the treatment,the circumference difference between the affected and healthy sides of the two groups were smaller than those before the treatment,those in the study group were smaller than those of the control group,and the differences were statistically significant(P<0.05).At 3 and 6 days after the treatment,the lower extremity venous patency scores in the two groups were lower than those before the treatment,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).At 7 days after the treatment,the levels of FIB and D-D in the two groups were lower than those before the treatment,the levels of FIB and D-D in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications/adverse reactions between the two groups(P>0.05).Conclusions:Angiojet mechanical thrombectomy in the treatment of the patients with acute lower extremity deep venous thrombosis can reduce the circumference difference between the
