摘 要:目的:探讨与分析肿瘤骨转移患者的99mTc-亚甲基二膦酸盐(MDP)骨显像骨代谢半定量参数与骨密度的相关性。方法:回顾性分析2021年1月—2022年12月福建医科大学附属宁德市闽东医院收治的168例恶性肿瘤骨转移患者临床资料,判断骨显像诊断骨质疏松的价值,进行骨显像骨代谢半定量参数和骨密度的相关性分析。结果:骨密度测量结果显示,在168例患者中,68例为骨质疏松(骨质疏松组);骨质疏松组的腰椎/股骨干每像素平均放射性计数(ROI)比值和股骨颈/股骨干ROI比值都高于非骨质疏松组,差异均有统计学意义(t=12.575、14.015,P<0.05);骨质疏松组的腰椎骨密度、股骨颈骨密度与非骨质疏松组对比都有显著降低,差异均有统计学意义(t=12.575、11.199,P<0.05);在肿瘤骨转移骨质疏松患者中,Pearson分析显示腰椎/股骨干ROI比值、股骨颈/股骨干ROI比值与腰椎骨密度、股骨颈骨密度呈负相关(r=-0.789、-0.697、-0.706、-0.745,P<0.05)。结论:恶性肿瘤骨转移骨质疏松症患者的99mTc-MDP骨显像骨代谢半定量参数与骨密度有较好的相关性,诊断骨质疏松症有较高的灵敏度,对肿瘤患者随访过程中合并骨质疏松症有一定的预测意义和应用价值。Objective:To investigate and analyze the correlation between semi-quantitative parameters of bone metabolism and bone mineral density on 99mTc-methylene diphosphate(MDP)bone imaging in patients with tumor bone metastases.Methods:The study retrospectively analyzed 168 patients with bone metastases of malignant tumors in the hospital from January 2021 to December 2022.The value of bone imaging for the diagnosis of osteoporosis was determined,and the correlation between semiquantitative parameters of bone metabolism in bone imaging and bone mineral density was analyzed.Results:Among 168 patients,68 cases were judged to be osteoporotic by bone density(osteoporosis group).The lumbar spine/femoral stem ROI ratio and the femoral neck/femoral stem ROI ratio in the osteoporosis group were higher than those in the non-osteoporosis group(t=12.575,14.015;P<0.05).The bone density of lumbar vertebrae and femoral neck in the osteoporosis group was significantly lower than that in the non-osteoporosis group,and the difference was statistically significant(t=12.575,11.199;P<0.05).In patients with osteoporosis with tumor bone metastasis,Pearson analysis showed that lumbar/femoral shaft ROI ratio,femoral neck/femoral shaft R0I ratio were negatively correlated with lumbar bone density and femoral neck bone density(r=-0.789,-0.697,-0.706,-0.745;P<0.05).Conclusion:Semi-quantitative parameters of bone metabolism in 99mTc-MDP bone imaging in osteoporosis patients with bone metastases from malignant tumors correlate better with bone density.The diagnosis of osteoporosis has a high sensitivity.It has certain predictive significance and application value for tumor patients with combined osteoporosis during follow-up.
