摘　　要：目的:探讨参苓白术散加减联合西药治疗对慢性肾衰竭(CRF)血液透析患者微炎症状态及贫血的影响。方法:选取2021年5月—2023年2月宜春市中医院收治的80例CRF血液透析患者作为研究对象,采用随机数表法将其分为对照组和观察组,每组各40例。对照组患者采用西药治疗,观察组患者在对照组的基础上加用参苓白术散加减治疗。对比两组患者的临床疗效、微炎症状态、贫血状况、免疫功能、中医证候积分及不良反应发生情况。结果:观察组患者临床总有效率高于对照组患者,差异有统计学意义(χ^(2)=6.275,P<0.05);治疗后,观察组患者肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、C反应蛋白(CRP)水平均低于对照组患者,差异均有统计学意义(t=6.738、4.301、3.817,P<0.05);治疗后,观察组患者血红蛋白(Hb)、红细胞比容(Hct)、红细胞(RBC)水平均高于对照组患者,差异均有统计学意义(t=11.930、8.109、13.395,P<0.05);治疗后,观察组患者CD4+、CD4+/CD8+水平均高于对照组患者,CD8+水平低于对照组患者,差异均有统计学意义(t=12.158、2.095、4.064,P<0.05);治疗后,观察组患者倦怠、面白无华、气短懒言、腰酸膝软、纳呆少食积分均低于对照组患者,差异均有统计学意义(t=5.597、4.991、8.474、8.340、7.057,P<0.05);两组患者均未发生不良反应。结论:参苓白术散加减联合西药治疗CRF血液透析患者,可改善其微炎症状态、贫血状况及免疫功能,缓解其临床症状,是一种安全、有效的治疗方案。Objective:To explore the effect of Shenling Baizhu Powder combined with Western medicine treatment on micro inflammatory status and anemia in hemodialysis patients with chronic renal failure(CRF).Methods:80 CRF patients who underwent hemodialysis treatment in the hospital from May 2021 to February 2023 were randomly divided into two groups by random number table method,with 40 patients in each group.The control group was treated with western medicine,while the observation group was treated with modified Shenling Baizhu Powder.The clinical efficacy,micro inflammatory status,anemia status,immune function,traditional Chinese medicine syndrome scores,and adverse reactions between the two groups were compared.Results:The total clinical efficacy rate of the observation group was lower than that of the control group(χ^(2)=6.275,P<0.05).The levels of tumor necrosis factorα(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)in observation group were lower than those in control group,with statistically significant difference(t=6.738,4.301,3.817;P<0.05).After treatment,the levels of hemoglobin(Hb),erythrocyte specific volume(Hct)and erythrocyte(RBC)in observation group were higher than those in control group,with statistically significant dfference(t=11.930,8.109,13.395;P<0.05).After treatment,CD4^(+),CD4^(+)/CD8^(+)levels in observation group were higher than those in control group,and CD8^(+)levels were lower than those in control group,with statistically significant difference(t=12.158,2.095,4.064;P<0.05).After treatment,patients in the observation group were lower than those in the control group in fatigue,white face,shortness of breath,lazy speech,lumbago and knee weakness and lile food,with statistically significant difference(t=5.597,4.991,8.474,8.340,7.057;P<0.05).No adverse reactions occurred in both groups.Conclusion:The combination of western medicine and Shenling Baizhu Powder can improve the micro inflammatory state,anemia status,and immune function of CRF hemodialysis patients;and alleviate clinical sympt
