吻合器痔上黏膜环切术对环状混合痔患者术后疼痛及肛门功能的影响

摘　　要：目的探究环状混合痔患者应用吻合器痔上黏膜环切术(PPH)的临床效果。方法选取2020年7月至2023年7月于我院行手术治疗的60例环状混合痔患者,采用随机数字表法分为对照组和观察组各30例。对照组采用传统的外剥内扎术(MMH)治疗,观察组采用PPH治疗。对比两组临床疗效、围术期指标、肛门出血次数及疼痛评分(VAS)、肛门功能及并发症。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组手术时间、创面愈合时间、住院时间及恢复工作时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。术后1 d、3 d,观察组出血次数少于对照组,术后3 d观察组VAS评分低于对照组,差异有统计学意义(P<0.05);术后1 d,两组VAS评分比较,无显著差异(P>0.05)。观察组并发症发生率为6.67%,低于对照组的26.67%,差异有统计学意义(P<0.05)。结论PPH在环状混合痔患者治疗中效果显著,对患者造成创伤较小,创面愈合时间较短,有助于患者尽早回归日常工作及生活,且并发症少,安全可靠,值得推广应用。Objectives To investigate the clinical effect of procedure for prolapse and hemorrhoids(PPH)in patients with circular mixed hemorrhoids.Methods A total of 60 patients with circular mixed hemorrhoids who underwent surgical treatment in our hospital from July 2020 to July 2023 were selected and divided into control group and observation group,with 30 cases in each group.The control group was treated with traditional milligan-morgan hemorrhoidectomy(MMH),and the observation group was treated with PPH.The clinical efficacy,perioperative indicators,frequency of anal bleeding and pain score(VAS),anal function and complications were compared between the two groups.Results The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).The operation time,wound healing time,hospitalization time,and recovery time of the observation group was significantly shorter compared to those of the control group,and the intraoperative bleeding was significantly less than that of the control group(P<0.05).On postoperative day 1 and 3,the observation group had less bleeding than the control group,and the VAS score of the observation group was significantly lower than that of the control group on postoperative day 3(P<0.05).On postoperative day 1,there was no statistically significant difference in VAS scores between the two groups(P>0.05).The incidence of complications in the observation group was 6.67%,which was significantly lower than the 26.67%in the control group(P<0.05).Conclusions PPHhas a significant effect in the treatment of patients with circular mixed hemorrhoids.It has less trauma and shorter wound healing time,which is helpful for patients to return to daily work and life as soon as possible.Moreover,it has fewer complications,and is safe and reliable,whichis worthy of popularization and application.
