立体定向微创手术治疗少量丘脑出血的有效性及安全性探讨

摘　　要：目的基于有效性、安全性两大方面,探讨丘脑少量出血外科手术治疗中,立体定向微创手术的应用价值。方法非随机选取临沂市中心医院于2021年1月—2023年12月收治的100例经颅脑CT检查确诊为少量丘脑出血的患者为研究对象,根据治疗方法不同分为保守治疗的对照组(n=50)和行立体定向微创手术治疗的观察组(n=50)。对比两组血肿体积、并发症、神经功能以及预后情况。结果观察组治疗后3、7 d的血肿体积均低于对照组,差异有统计学意义(P均<0.05)。观察组并发症总发生率为6.00%(3/50),低于对照组的20.00%(10/50),差异有统计学意义(χ^(2)=4.332,P<0.05)。观察组治疗后2周、治疗后4周的神经功能评分优于对照组,差异有统计学意义(P均<0.05)。观察组的预后情况优于对照组,差异有统计学意义(P<0.05)。结论少量丘脑出血应用立体定向微创手术可行,可改善患者血肿、神经功能和预后,减少并发症,有效性和安全性都较高。Objective Based on the two aspects of effectiveness and safety,this paper discusses the application value of stereotactic minimally invasive surgery in the surgical treatment of small thalamic hemorrhage.Methods A total of 100 patients with a small amount of thalamic hemorrhage diagnosed by craniocerebral CT examination in Linyi Central Hospital from January 2021 to December 2023 were non-randomly selected as the research objects.According to different treatment methods,they were divided into control group(n=50)with conservative treatment and observation group(n=50)with stereotactic minimally invasive surgery.The hematoma volume,complications,neurological function and prognosis were compared between the two groups.Results The hematoma volume of the observation group was lower than that of the control group at 3 d and 7 d after treatment,and the differences were statistically significant(both P<0.05).The total incidence of complications in the observation group was 6.00%(3/50)lower than 20.00%(10/50)in the control group,the differences were statistically significant(χ^(2)=4.332,P<0.05).The neurological function scores of the observation group were better than those of the control group at 2 weeks and 4 weeks after treatment,and the differences were statistically significant(both P<0.05).The prognosis of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Stereotactic minimally invasive surgery for a small amount of thalamic hemorrhage is feasible,which can improve the hematoma,neurological function and prognosis of patients,reduce complications,and has high efficacy and safety.
