摘　　要：目的探讨超脉冲二氧化碳点阵激光联合外用曲安奈德和氟尿嘧啶治疗增生性瘢痕的临床疗效和安全性。方法选取2021年1月至2023年1月福建医科大学附属三明市第一医院收治的80例增生性瘢痕患者,按照治疗不同分为2组,每组40例,对照组患者采用单纯激光治疗,试验组患者采用激光联合术后即刻局部外用曲安奈德和氟尿嘧啶混合液治疗,两组患者每个月治疗1次,2次治疗间隔1个月,共接受8次治疗。比较两组患者治疗前、治疗后2个月、4个月、6个月及8个月后温哥华瘢痕评估量表(VSS)评分、患者瘢痕评估量表(PSAS)评分。两组患者治疗前、治疗4个月、8个月分别行皮肤病生活质量指数(DLQI)评分。结果两组患者均完成了全部治疗,随着治疗次数的增加,两组患者VSS评分均逐步降低,PPSSAASS评分逐渐降低。试验组VSS评分及PSAS评分均低于对照组,差异有统计学意义(P<0.05)。试验组生活质量指数(DLQI)评分低于对照组,差异有统计学意义(P<0.05)。结论采用超脉冲二氧化碳点阵激光联合外用曲安奈德和氟尿嘧啶混合液治疗疗效更好,安全性较高,值得临床推广。Objective To evaluate the clinical effificacy and security of superpulsed carbon dioxide laser combined with topical triamcinolone and flfluorouracil in the treatment of hypertrophic scars.Methods A total of 80 patients with hypertrophic scar admitted to Sanming First Hospital Affifiliated to Fujian Medical University from January 2021 to January 2023 were selected and divided into two groups according to different treatments,40 cases in each group.The control group was treated with simple laser treatment,and the experimental group was treated with laser combined with triamcinolone acetonide and flfluorouracil mixture immediately after surgery.The two groups of patients were treated once a month,with an interval of 1 month,and received a total of 8 treatments.The Vancouver Scar Assessment Scale(VSS)score and patient scar assessment scale(PSAs)score were compared between the two groups before treatment,2 months,4 months,6 months and 8 months after treatment.The dermatological quality of life index(DLQI)scores were performed before treatment,4 months and 8 months after treatment in the two groups.Results Both groups of patients completed all treatments.With the increase of treatment times,the VSS score and PSAs score of both groups gradually decreased.The VSS score and PSAs score of the experimental group were lower than those of the control group,and the difference was statistically signifificant(P<0.05).The DLQI score of the experimental group was lower than that of the control group,and the difference was statistically signifificant(P<0.05).Conclusions The combination of superpulse carbon dioxide laser with external triamcinolone and flfluorouracil has better curative effect and higher safety,which is worthy of clinical promotion.
