吸脂法辅助腋窝入路腔镜手术治疗男性乳腺发育的临床体会

摘　　要：目的探讨吸脂法在经腋窝入路腔镜手术治疗男性乳腺发育的临床效果与安全性分析。方法回顾性分析2017年1月至2023年12月于鹤壁市人民医院确诊为男性乳房发育的36例患者的临床资料,分为对照组和试验组,每组18例。其中对照组行经腋窝入路腔镜手术,试验组行吸脂法辅助经腋窝入路腔镜手术切除发育腺体,比较两组患者围手术期临床指标,术后并发症,乳房外形评价,患者满意度评价等指标。结果两组患者术中出血量,拔管时间,住院总时间比较,差异无统计学意义(P>0.05);试验组单侧手术时间略长于对照组,但差异无统计学意义(P>0.05);术后6个月乳房外形评价,患者满意度评价均优于对照组,差异有统计学意义(P<0.05)。结论吸脂法辅助经腋窝入路腔镜手术治疗男性乳腺发育,虽手术时间略长,但可行性好,安全性高,并发症少,整体费用适中,乳房外形评价高,患者满意度好,更适合在有条件的医院积极推广应用。Objective To explore the clinical effect and safety of liposuction in endoscopic surgery via the axillary approach for the treatment of gynecomastia in males.Methods A retrospective analysis was performed on the clinical data of 36 patients diagnosed with gynecomastia in our department from January 2017 to December 2023.They were divided into the control group(n=18)and the experimental group(n=18).The control group underwent endoscopic surgery via the axillary approach,while the experimental group underwent liposuction-assisted endoscopic surgery via the axillary approach to resect the developed glands.The perioperative clinical indicators,postoperative complications,breast shape evaluation,and patient satisfaction evaluation were compared between the two groups.Results There was no signifificant difference in intraoperative blood loss,extubation time and total hospital stay between the two groups(P>0.05);The unilateral operation time of the experimental group was slightly longer than that of the control group,but the difference was not statistically signifificant(P>0.05);At 6 months after operation,the breast shape evaluation and patient satisfaction evaluation were better than those of the control group,and the difference was statistically signifificant(P<0.05)Conclusions Although the operation time is slightly longer,liposuction-assisted endoscopic surgery via the axillary approach for the treatment of gynecomastia in males has good feasibility,high safety,few complications,moderate overall cost,good breast shape evaluation,and high patient satisfaction.It is more suitable for active promotion and application in hospitals with the necessary conditions.
