摘 要:目的分析双任务训练模式结合Bobath技术对脑卒中后偏瘫患者上肢功能及脊髓运动神经元兴奋性的影响。方法择取2020年11月至2022年11月我院收治的90例脑卒中后偏瘫患者作为研究对象,随机将其分为对照组和观察组,每组45例。对照组采用常规康复训练,观察组在对照组基础上加双任务训练模式结合Bobath技术进行康复训练。比较两组的干预效果。结果干预后,观察组的改良Ashworth量表(MAS)评分低于对照组,简化Fugl-Meyer运动量表(FMA)、躯干控制测试(TCT)评分明显高于对照组(P<0.05)。干预后,观察组的偏瘫侧下肢H波最大波幅(Hmax)和M波最大波幅(Mmax)、Hmax/Mmax均低于对照组(P<0.05)。干预后,观察组的WHO生活质量量表简表(WHOQOL-BREF)各维度评分均高于对照组(P<0.05)。结论双任务训练模式结合Bobath技术可加速脑卒中后偏瘫患者上肢功能恢复,降低其脊髓运动神经元兴奋性,改善临床症状,提高生活质量。Objective To analyze the effects of dual-task training mode combined with Bobath technique on upper limb function and spinal motor neuron excitability in patients with hemiplegia after stroke.Methods A total of 90 patients with hemiplegia after stroke admitted in our hospital from November 2020 to November 2022 were selected as study objects and randomly divided into control group and observation group,with 45 cases in each group.The control group was given with routine rehabilitation training,and the observation group was given with dual-task training mode combined with Bobath technique on the basis of the control group.The intervention effects of the two groups were compared.Results After intervention,the Modified Ashworth scale(MAS)score of the observation group was lower than that of the control group,and the simplified Fugl-Meyer Assessment(FMA)and Trunk Control Test(TCT)scores were significantly higher than those of the control group(P<0.05).After intervention,the maximum amplitude of H wave(Hmax),the maximum amplitude of M wave(Mmax)and Hmax/Mmax of hemiplegic lower limbs in the observation group were lower than those in the control group(P<0.05).After intervention,the scores of each dimension of the WHO Quality of Life Brief Scale(WHOQOL-BREF)in the observation group were higher than those in the control group(P<0.05).Conclusion The dual-task training mode combined with Bobath technique can accelerate the recovery of upper limb function in hemiplegic patients after stroke,reduce the excitability of spinal motor neurons,improve clinical symptoms and enhance the quality of life.
