一期前后路联合手术治疗下颈椎骨折脱位研究

摘　　要：目的 探究一期前后路联合手术治疗下颈椎骨折脱位的效果,为下颈椎骨折脱位患者探寻更好的手术方式。方法选取2020年1月—2021年12月滕州市中心人民医院收治的42例下颈椎骨折脱位接受一期前后路联合手术治疗的患者作为研究对象,观察手术时间、术中出血量、住院时间、疼痛程度、颈椎功能、脊椎功能。结果 42例患者均成功完成手术,平均手术时间为(125.06±24.53)min,平均术中出血量为(452.21±36.97)mL,平均住院时间为(9.71±1.95)d。患者术后1个月VAS评分明显低于术前,JOA评分明显高于术前,末次随访ASIA分级D级和E级占比高于术前,差异均有统计学意义(P<0.05)。结论 在下颈椎骨折脱位的治疗中,一期前后路联合术式的应用能够获得满意的效果,不仅能有效缓解疼痛症状,促进颈椎功能与脊髓功能的恢复,还能实现脊柱重建、环脊髓减压的目标,可以作为治疗严重下颈椎骨折脱位的重要方法。Objective To explore the effect of one-stage anterior and posterior combined surgery for the treatment of lower cervical spine fractures and dislocations,and to find a better surgical method for patients with lower cervical spine fracture and dislocation.Methods A total of 42 patients with lower cervical spine fracture and dislocation received the treatment of one-stage anterior and posterior combined surgery in Tengzhou Central People's Hospital from January 2020 to December 2021 were selected as the study subjects.The time of surgery,intraoperative bleeding volume,hospital stay,degree of pain,cervical function and spinal function were observed.Results All 42 patients successfully completed the surgery,with an average surgical time of(125.06±24.53)minutes,an average intraoperative bleeding volume of(452.21±36.97)mL,and an average hospital stay of(9.71±1.95)days.The VAS score of patients after surgery one month was significantly lower than that before surgery and JOA score was significantly higher than that before surgery,the proportion of D grades and E grades in ASIA grading at the last follow-up were higher than those before surgery,and the differences were statistically significant(P<0.05).Conclusion In the treatment of lower cervical spine fractures and dislocations,the application of onestage anterior and posterior combined surgery could achieve satisfactory effects,not only effectively alleviating pain symptoms,promoting the recovery of cervical and spinal cord functions,but also achieving the goals of spinal reconstruction and decompression of the circumspinal,and could also be an important means of treating severe lower cervical spine fractures and dislocations.
